General Summary: The Care Coordinator (CC) Is Primarily Responsible for Coordinating and scheduling field staff. S/he will ensure each Touching Angels (TA) client is matched with the best caregiver(s) to meet their preferences and abilities in-order to foster quality Home-Care services.

Duties and Responsibilities: This position requires s/he to maintain a high-volume workload within a fast-paced environment.

Essential Job Functions:

  • Answer Incoming calls to dispatch caregivers on available cases to ensure coverage.
  • Maintains a current awareness of the abilities and limitations of each field staff member.
  • Consistently counsel field staff members to provide exceptional Home Care Services.
  • Demonstrates a thorough knowledge and adherence to company policies and Procedures; Holds staff accountable for carrying out all policies and procedures as outlined in the manuals.
  • Effectively utilizes nurses when assigning cases.
  • Demonstrates an ability to identify and solve problems; Uses Initiative and good Judgment to reach quality decisions.
  • Maintains an excellent rapport with clients and caregivers and effectively promotes harmonious interpersonal relationships.
  • Provides leadership through constant planning, organizing, coordinating, monitoring, and thoroughly evaluating the services given to clients associated with Touching Angels.
  • Coordinate work schedules by allocating field staff in Shifts and Positions.
  • Be Primary Point of Contact with Patient Family Regarding Staffing Schedules

Care Coordination duties:

· Coordinate and ensure the scheduling of client cases daily. This includes selecting the appropriate caregiver, communicating with the family or client on an ongoing basis regarding scheduling matters.

· Assist HR with recruiting and orientation in-order to prepare Nurse Skill Set to Meet Client's Expectations.

· Anticipates Staffing Needs and Works with The HR To Ensure That an Adequate Pool of Employees Is Maintained.

· Responsible for collaborating with the Executive Leadership team in Performance Management of The Field Staff.

Requirements

· High School Diploma or Ged

· One (1) Year Previous Agency Staffing or Recruiting Experience

· Must Be Able to Work After hours for On-Call Schedule

· Proficient Microsoft Office – Excel, Outlook, Word Etc.

· Proficient Typing Skills – 35 wpm minimum.

· Active CNA License (preferred)

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
