• Develops and maintains nursing practice standards of care for a specific transplant population.
• Serves as a liaison and provides consultation and education services to support standards of care.
• Identifies and resolves clinical issues relating to the transplant patient.
• Coordinates the collection of clinical data and patient samples for research studies.
• Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning.
• Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.
• Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, arranging for long-range beepers, outpatient treatments, and subsequent hospital admissions.
• Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.
• Participates in daily patient rounds with transplant team.
• Participates in collaborative, interdisciplinary research activities related to transplant patients.
• Performs related responsibilities as required.
  
  
  

MINIMUM QUALIFICATIONS:

• A bachelor's degree in nursing or ADN with four years of recent, clinical, medical-surgical nursing experience in the specialty of transplantation and/or outpatient care coordination, with a minimum of two years’ experience in the care of the transplant population preferred.
• All nurses employed in roles requiring an unencumbered nursing license must have a valid, active license or temporary permit approved by the Georgia Licensing Board.
• BLS certification required.

PHYSICAL REQUIREMENTS (Medium):20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.
