Resident Care Coordinator

Description:Summary:
The Resident Care Coordinator is responsible for assisting the Wellness Director with coordinating the care of our residents, monitoring care, care plans, and resident medications, assessing residents’ needs, communicating with residents and their families and ensuring state compliance and regulations. This position actively participates in passing medications to our residents regularly, assists with care and monthly vital checks for residents, and assists with training for team members.
Essential Duties & Responsibilities:
- Assure resident care and medication supervision/administration is carried out by Medication Supervisors in accordance with company policies and procedure and within State regulations.
- Perform monthly, quarterly, and yearly assessments per state regulations and/or as needed per incident or change in condition.
- Assist with medication and treatments as prescribed by physician. Monitors response to existing treatment plans, aware of prominent medication interactions and side effects; monitors residents for occurrence of such. Adheres to community’s infection control policies and procedures relevant to medication administration.
- Provides general oversight of Memory Care resident care and safety; Makes routine rounds in the Memory Care observing, assisting, and directing care as needed.
- Provides Memory Care Engagement to residents and makes monthly Memory Care Engagement Calendar.
- Communicate and correspond promptly and compassionately to family and/or responsible party and resident concerns while upholding best interest of resident and complying with company policy and State licensing requirements.
- Advise Avanti team on daily living activities per state polices and guidelines.
- Consult with and monitor services of pharmacy consultant, in conjunction with Wellness Director.
- Monitor resident care and medication assisting with team member performance and conduct retraining when needed, in conjunction with the Wellness Director.
- Monitor and improve nurse call system and response times.
- Coordinate and communicate resident care with third party providers including but not limited to home health, hospice, physicians, and pharmacies.
- Assist with maintaining care plans and resident incident reports and assessments as required by State regulations for entry into Electronic Medical Record; ensure team enters medical services on service plan in Electronic Medical Record.
- Audit medication and health records as well as medication carts.
- Maintain all required documentation necessary to ensure that residents’ medications are handled properly and timely in compliance with prescribers, company policy and state regulations.
- Maintain residents’ records daily in a timely manner and in accordance with company policy and procedure. This documentation includes but not limited to: medication distribution, incident reports and promptly reporting all changes to the Wellness Director.
- Maintain records of medications released to and returned by any resident or family member in compliance with company policy and state regulations.
- Maintains records of controlled drugs in compliance with company policy and state regulations.
- Maintain the cleanliness and organization of the medication rooms and medication carts.
- Conduct monthly self-medication review for residents who keep medications in their suites.
- Assist with interviewing; train and/or coordinate training; and assist with monitoring performance of team members.
- Assist with monthly in-service and training sessions for team.
- Attend mandatory monthly in-service and safety meetings.
- Assist with conduction performance counselling, both verbal and written, in conjunction with the Wellness Director.
- Be available on-call 24 hours for emergencies.
- May act as supervisor during the night shift.
- Perform other duties as requested by the Executive Director.

- Responsible for following and advocating all government and state health and safety policy regulations.
- Perform job duties in compliance with company policy and procedures and state licensing regulations
- Adhere to confidentiality policy regarding information concerning residents and employees, HIPPA.
- Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information.
- Be observant and aware of any situations or occurrence in the community that may pose a safety hazard to residents or staff. Report such observation immediately to the Wellness Director

Additional Responsibilities:
- Attend all required meetings and in-services.
- Be observant and aware of any situations or occurrence in the community that may pose a safety hazard to residents or staff. Report such observation immediately to the Executive Director.
- Must remain on duty and provide essential functions for residents until shift replacement arrives.
- Adhere to company policy for time and attendance.
Physical/Environmental Demands:
- Moving about on foot to accomplish tasks and support residents, particularly for long distances.
- Standing, particularly for sustained periods of time.
- Exerting up to 50 pounds of force occasionally to push, lift, carry, pull, or otherwise move objects.
- Must have clear and color vision and the ability to adjust focus when necessary
- Must be able to reach with arms in order to assist and support residents
- Must meet the general health requirements set forth by the communities, which may include a medical and physical examination.
- Must be able to lift, turn, move, position, and transport residents alone or with assistance.
- Must be able to assist in the evacuation of residents & exchange accurate information in these situations.
- Must have the ability to hear and react to Exit Door Alarms and Emergency phone rings.
- Must pass physical requirement, TB test, background check, drug test and maintain required immunizations.
Desired Skills and Experience:
- High School Diploma or equivalent is required.
- Bachelor’s degree in nursing or related field required.
- Must hold a nursing license in good standing with the State of Issuance as an LVN or LPN.
- Training and experience in caring for those with Alzheimer’s/Dementia
- Passion for working with and understanding residents with Alzheimer’s or Dementia.
- Two to five years of prior assisted living or senior care experience preferred.
- Ability to work flexible schedules in 24-hour operation including weekends.
- Proficient in written and verbal English.
- Demonstration of ability to establish long-term relationships.
- Must have compassion for and a desire to work with the older adult population.
- Must exhibit and promote a high level of customer service, hospitality, curiosity and friendliness towards all prospects, residents, visitors, employees and the overall community.
- Ability to communicate effectively in writing and orally, speaking the primary language of the residents.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
- Basic personal computer skills including electronic mail, word processing, spreadsheet, internet etc.
- Ability to easily learn, understand, and adapt to new or unfamiliar technology.
Environmental Adaptability:
- Works primarily indoor in a climate-controlled setting
- Possible exposure to unpleasant odors
- Possible exposure to chemicals as identified in the MSDS Manual
- Possible exposure to verbal aggression from vendors, guests, residents and/or family members.
- Continuous exposure to clients and/or family members who may be under stress
Working Hours
- Must be available to rotate on-call weekends with the Wellness Director
- Must be available to work weekends, extended shift hours, holidays, on short notice, and when required by management.
- Must be Flexible with work schedule and related job assignments.
