Sheppard Pratt's community residential programs offer adults with mental illness a range of housing options in apartments, townhouses and single family homes throughout Maryland. Services are also provided to individuals in their own homes. The programs honor the dignity of the individuals they serve and are respectful of personal choices. Services are person-centered, strength-based, and recovery-oriented, and are provided at a frequency and intensity that meets each resident's individual needs. The programs, which are nationally accredited and designed to facilitate independence and community engagement, use leading-edge and evidence-based practices that take into account each person's strength, needs, desires, health and safety. The community residential programs are seeking compassionate people to provide services, supports, and interventions that assist individuals in living in the community and achieving their personal goals. Responsibilities include care coordination and resource management, money management planning, medication monitoring, and assisting clients with grocery shopping, meal preparation, housekeeping, self-care, socialization, transportation, and advocacy. Direct care employees work as part of collaborative service teams that develop and implement individual service plans based on the assessed needs of the individuals served. Additional responsibilities include documenting and reporting services, client progress and ongoing needs.
