I) Job Purpose

QMHP-CS (Qualified Mental Health Professional-Community Service)
The position of QMHP-CS is a direct care position with the expectation that at least 50% of his/her work time is spent face to face with the individuals that MHMR of Tarrant County serves. The position has specializations that include functioning as a Rehabilitative Case Manager (RCM, RCM-I) or a Clinical Coordinator (CC). A QMHP-CS functions as a part of the treatment team in the clinic to which he/she is assigned, and provides assessment, recovery planning, and ongoing treatment in order to assist individuals with severe and persistent mental illness to gain the skills, resources, and supports necessary to attain their individualized recovery goals. Services may be provided in the clinic or in the community. The specific job function designation (RCM, RCM-I, CC) determines what percentage of time the QMHP spends in the clinic or the community. Providing services in the community entails travel (often in the QMHP’s own vehicle) and work throughout Tarrant County, in a variety of neighborhood and home conditions.


II) Essential Functions

QMHP’s are evaluated routinely and as needed based on the following expectations beginning 90 days after date of credentialing:

A. Prior to providing billable services, QMHP completes or coordinates the completion of assessments and recovery plans, and verifies completion of authorization and Determination of Medical Necessity.

Standard:

  • At least 95% of billable services provided by QMHP meet billing standards


B. Contact assigned individuals within 3 business days after the assignment is made.

Standard:

  • Contacts/attempts to contact within 3 business days are documented in the assigned individuals’ charts 100% of the time.


C. Spend at least 50% of working hours face to face with the individuals served by MHMR of Tarrant County.

Standard:

  • Each month, QMHP’s face to face time with individuals served is no lower than 40% of hours worked.
  • There are no more than 2 consecutive months during which QMHP’s face to face time with individuals served is lower than 50% of hours worked.
  • Each FY quarter, QMHP’s face-to-face time with individuals served average 50% of total hours worked.


D. Provide billable services based on clinical role.

Standard:

  • RCM face-to-face services provided are billable at least 75% of the time.
  • RCM-I face-to-face services provided are billable at least 25% of the time.
  • CC face-to-face services provided are billable at least 5% of the time.


E. Complete documentation of services provided on a daily basis, and turn in documentation and Service Activity Log (SAL) to designated location by 10 am the next business day.

Standard:

  • All SALs must be completed, processed (including corrections), and entered into the agency’s data system no later than within the time frame designated by current division policy.


F. Utilize Collaborative Documentation when providing services to an individual.


G. Attend and participate in all Recovery Team Meetings

Standard:

  • QMHP obtains prior approval from his or her Team Lead or Program Manager for every Recovery Team Meeting not attended.


H. Maintain credentialing as a Qualified Mental Health Professional.

Standard:

  • No Relias training is 60 days or more overdue at any time.
  • QMHP maintains clinical supervision requirements at all times.


I. Maintain timekeeping and travel records on a daily basis, ensuring final records are turned in and approved before the close of each pay period or as requested by supervisor.

Standard:

  • QMHP has no more than two missed punches (clock-in or clock-out) in the timekeeping system in any given pay period.
  • QMHP notifies his or her supervisor immediately if a missed punch occurs.


J. Complete additional duties as assigned.

L. Perform other job duties or responsibilities as requested or assigned.


III) Knowledge of Laws, Policies/Procedures, Skills, and Abilities

  • Applicable sections of Texas Administrative Code, Third Party Payer Requirements, MHMR of Tarrant County Policies and Procedures, Mental Health Code, Medicaid Procedures, Compliance Plan, MHMR of Tarrant County Employee Handbook, HIPAA, and applicable Federal standards.
  • QMHP demonstrates effective organizational, time management, and communication skills.
  • QMHP is self-motivated and self-directed demonstrating the ability to work autonomously.
  • QMHP provides excellent customer service both within and outside the agency.
  • QMHP demonstrates the ability to engage individuals in services while maintaining appropriate professional boundaries.
  • QMHP demonstrates the ability to work in a non-judgmental manner with individuals and coworkers whose belief systems may be based on values different from those of the QMHP.
  • QMHP is able to communicate clearly and concisely both orally and in writing.
  • QMHP demonstrates the ability to work as a member of a multidisciplinary treatment team.
  • QMHP demonstrates the ability to accept direction and make constructive use of professional and clinical supervision.


IV) Internal & External Customer Service

This position requires extensive internal and external contacts with clients, family members, significant others, inter-agency staff, service providers, and practitioners. The QMHP accomplishes communication with exceptional written and verbal skills, by answering phone calls and emails within one business day, and by conducting themselves professionally in all interactions. Other contacts may include, but are not limited to: John Peter Smith Hospital, Family Services, Social Security Administration, Department of Assistive and Rehabilitative Services (formerly TRC), Department of Regulatory and Protective Services, other area hospitals, local Police Department(s), area United Way Agencies, local housing authority and/or local Transportation Authority.


V) Travel

This position requires driving to and from locations for work and/and may require overnight traveling. Employees are required to have a license in good standing and be able to be covered by agency liability insurance. If personal vehicle is utilized for work travel, then maintaining personal auto insurance coverage is required.
