Position Summary
- Functions as part of the Nurse Residency program cohort with a 2 year commitment to Hawaii Care Choices. The Nurse Resident participates under supervision and guidance in providing skilled nursing to hospice patients.
Duties & Responsibilities
- Under supervision and with guidance, completes the nursing assessment of the patient, including assessment of the care givers needs, and develops the plan of care in collaboration with the assigned mentor and interdisciplinary team.
- Provides nursing services to patients and care givers in compliance with federal, state, and Hospice regulations, initiating and coordinating utilization of services that best benefit the patient and care givers.
- Documents all nursing and case management in accordance with Hospice protocol, and keeps attending physicians informed of changes in patient’s condition.
- Empowers and supports care givers and patients through education of the disease process and prescribed care regimens.

- Comfortable in maintaining professional boundaries.
- Interest and desire to give hospice care with a special sensitivity to the needs of the seriously ill population.
- Ability to provide health care services in private residences, along with handling and administering controlled and prescribed medications.
Educational and Experience Requirements
- Graduate of a registered nursing curriculum; BSN or higher preferred.
- Possession and maintenance of a valid Hawai'i State nursing license.
Hours
- Primarily Monday to Friday, 8:00 a.m. to 4:30 p.m., with possibility of evening, weekend and holiday work.
- Assists in maintaining the 24 hour on-call coverage as needed.
