Registered Nurse Education and Experience Graduate of an accredited school of nursing. Current licensure as Registered Nurse in the State of Michigan required. BSN preferred. One year clinical experience in Med-Surg nursing. Home Care or Critical Care nursing preferred. Qualifications Possesses excellent observational skills and nursing judgment. Possesses good oral and written skills. Possesses and maintains good physical stamina/mental health. Supervised by Clinical Manager Administrator Workers Supervised Assists in the supervision of Home Health Aide Services Responsibilities: 1. Provides skilled professional nursing care to the patient as prescribed by the physician and in 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Performs within the limits of preparation and experience. 4. Accepts the responsibility to be the Case Manager of home care patients when taking 5. Maintains an ongoing responsibility for assigned caseload as well as coordination of all services 6. Seeks and obtains sufficient information necessary to function effectively in the home care 7. Assesses and evaluates patients status by: A. Writing and initiating the plan of care. B. Regularly reevaluating patient needs. C. Revising plan of care as necessary. D. Involving patient and family/caregiver in care planning. 8. Initiates and maintains implementation of orders prescribed by the physician in the physician's 9. Utilizes appropriate preventative, therapeutic and rehabilitative nursing procedures and 10. Maintains patient confidentiality at all times. 11. Promotes comprehensive care to patients by utilizing services available ie, PT, OT, MSW, 12. Observes the patient for changes in condition or needs, maintaining ongoing communication 13. Assists the patient with the activities of daily living, facilitates the patient's efforts toward self- 14. Assists patient and family/caregiver in maintaining a safe home environment. 15. Maintains excellent infection control practices per OSHA guidelines and Agency policies. 16. Supervises care given by the Home Health Aides. 17. Utilizes Clinical Manager or Administrator as resources when problems or questions occur in 18. Reports to Clinical Manager and/or Administrator to verify SOC's, recerts, and hospitalizations 19. Abides by and promotes Agency administrative policies. 20. Complies with accepted professional standards and principles. 21. Maintains professionalism in appearance and in communication with all outside contacts and 22. Completes, submits and maintains accurate and relevant clinical notes on a timely basis per 23. Attends all mandatory agency staff meetings, case conferences and educational offerings. 24. Uses supplies and equipment effectively and efficiently. 25. Possesses and maintains current CPR certification and Tuberculin testing.
