Description/Job Summary RHD Grubb Rd Group Home is a residential program for 5 adults with serious mental illness. Services focus on skill-building in symptom management and community living, maximizing quality of life, and reducing the need for inpatient hospitalizations. The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD's multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness. Responsibilities/Duties Support residents in establishing and maintaining optimal physical and mental health Assess mental and physical health needs/concerns on a daily basis Provide counseling as needed to residents on chronic health care issues Facilitate psychoeducational groups in support of mental and physical health Maintain standards for medication documentation, ordering, storage, and disposal that is in accordance with DSAMH, CARF, LLAM, RHD regulations Attend weekly treatment teams to collaborate with psychiatrist in supporting the mental Schedule, attend, and follow up on medical appointments in accordance with DSAMH, CARF, RHD regulations Provide education to staff on health issues relevant to the population and residential setting Communicate with pharmacy, medical personnel, and other community supports in on an ongoing basis to assure resident needs are met Provide support in urgent or crisis situations (in person or via telephone) Required Experience Required Education Required Qualifications Must be a Delaware Licensed Registered Nurse Driver's License required One year working with the mentally ill population Ability to multi task and work cooperatively as part of a multidisciplinary team that includes CNA's, clinicians, psychiatrists LLAM trained RN's Residential experience preferred Resources for Human Development is an equal opportunity employer.
