Description/Job Summary The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD's multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness. The Community Transitional Recovery Program (CTRP) Nurse provides medical and psychiatric monitoring and direct care support via individual and group sessions. The services are designed for individuals diagnosed with a severe and persistent mental illness and having met the criteria for participation in the CTRP program.The responsibilities of the nurse include but are not limited to conducting the nursing assessment of the medical and psychiatric status of the consumers, Provide support as a liasion to community based medical providers regarding illness, medication, symptom management and activities and interventions to enhance well-being. Salary: $27.90 per hour Responsibilities/Duties Required Experience 2 years' experience working in a medical facility or mental health setting is preferred. Understanding of and interest in working with persons with serious mental illness in the community and home environment. Required Education Required Qualifications Program Summary RHD North Carolina's Community Transitional Recovery Program (CTRP) provides short term increased supervision and support to individuals who have high and complex needs exiting state institutions or hospitals to further assess and prepare them for community-based living. CTRP provides a less restrictive alternative to hospitalization that further promotes recoveryand community integration. CTRP creates a continuum of care from crisis to community living with links to community services and supports. Resources for Human Development is an equal opportunity employer.
