REFERRAL SPECIALIST JOB DESCRIPTION
*

The SWF Referral Specialist reports to the Program Director. The Referral Specialist is responsible for collaborating with the Second Wind Fund team to create, implement, and execute the referral process.

Responsibilities include but are not limited to:
- Implementing and enforcing program-related policies and procedures related to the client referral system – primarily communicating with qualified referral sources (QRS) seeking a SWF referral.
- Playing a significant role in collection of referral data and ensuring the quality, consistency and accuracy of the data with the most relevant tools available.
- Guiding the QRS’s through the referral process and provide any and all follow up.
- Guiding interested QRS (Qualified Referral Sources) through the training and onboarding process. Additionally, as necessary through any other processes that relate to becoming a SWF QRS.
- Strengthening the QRS network by maintaining clear communication and follow up, including the management QRS data accuracy.
- Playing a key role in following up and engaging with youth/families who receive SWF referrals to ensure program success through connection.
- Contribute to monthly QRS newsletter.
- Manage QRS benefits program to build community and work towards our mission of better supporting youth and those that serve youth through continued trainings, community support, etc.
- Assisting with implementation of program department improvements, for example, technology, program culture shift, etc.
- Playing a key role in ensuring that all program objectives are met and that outside parties (ie. QRS, providers, the community) who interface with SWF all receive a high-quality experience.
- Contributing to providing program updates to all relevant parties.
- As needed, assisting with outreach activities including event coordination for all program-related events.

The Program team is the primary lead on communications with program providers, school mental health staff, community mental health agencies, and other referral sources and resources on therapy-related issues. The Program team works regularly with other agencies and organizations across the state of Colorado, including some national organizations (e.g. AFSP, Trevor Project). In addition to the above program responsibilities, the Referral Specialist may also be responsible for other tasks related to programming, such as grant administration and reporting, and representing the organization at community events.

The Referral Specialist will also perform other tasks as assigned by the Program Director and/or Executive Director, at times to include tasks related to development and the administration of the organization. Duties are performed in an office setting (work from home during time of COVID-19), utilizing normal office equipment. Driving to various locations may be required and all relevant business-related expenses are reimbursed, including mileage.

- Degree in social work, psychology, counseling or related area
- Experience working in a program department in mental health, human services or related fields
- Experience navigating the mental health system
- Experience with Zoho, or other similar databases
- Sensitivity to HIPAA-related issues
- Experience in handling mental health crisis and suicide related situations and phone calls
- Demonstrated ability to move quickly from one topic or task to another
- Ability to process significant amounts of information and prioritize to best support programmatic growth
- Ability to develop expertise on suicide related issues as well as resources related to mental health services
- Experience working in a nonprofit setting
