Job Description: The Quality Care Coordinator will conduct outreach to our specific member population to encourage healthy behaviors through innovative telephonic communications. Performance driven on all provided targeted lists received by our Health Care partners. The Quality Care Coordinator will also enter data into our data base for all CPT II codes being performed. The Quality Care Coordinator will schedule and follow up on clinical care needs for patients as it relates to Healthcare Effectiveness Data and Information Set (HEDIS) and other quality STAR initiatives. The position contributes to the organization's drive to improve the overall experience of patient care, including the quality of health, and work to reduce the cost of care across the population served.
