Company Description
The OnTrackNY Program is an innovative treatment program that serves adolescents and young adults who are within 2 years of experiencing the onset of non-affective psychosis. The Psychiatric Nurse Practitioner will work 0.50 FTE (17.5 hours per week) as part of a small multidisciplinary team providing coordinated and assertive treatment in the earliest stages of illness. The team PNP engages the consumer in shared decision making about medication and the next steps in medication treatment.
This position requires proof of full vaccination within 30 days of hire.
We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This job requires work in our offices and/or the community

Job Description
- Assess the need for medications targeting specific symptoms of psychosis and other co-morbid conditions such as depressed mood and anxiety.
- Systematically manage and monitor dose changes and side effects in order to ensure the safety and tolerability of these medications.
- Will provide ongoing diagnostic assessment.
- Will provide ongoing safety assessment and crisis interventions.
- Will provide ongoing assistance and support for preventing and coping with relapse.
- Will provide psychoeducation regarding diagnosis and medications for participants and family members.
- Will go out into the field if needed.
- Will participate in weekly team meetings as well and mandated OnTrack training
- Will be available by phone, after hours (nights and weekends)
- Conduct metabolic monitoring (tracking blood pressure, weight, and body mass index) for patients
- Obtain and ensure processing of lab work for patients unable to have lab work done elsewhere in the community
- Obtain lab results when patients have labs drawn by outside providers
- Administer depot injections to patients on-site and in the community when patients are not attending appointments in the clinic
- Coordinate clozapine monitoring (obtaining lab results, reviewing them, and faxing prescriptions)
- Assess for psychiatric symptoms and response to medications
- Assess for medication side effects and educating patients on how to manage side effects
- Work with patients and families to develop strategies to enhance medication adherence
- Coordinate care for medical issues, including providing referrals to community medical providers, scheduling appointments, and liaising with medical providers to obtain patients’ records
- Conduct quarterly health screenings for patients
- Provide wellness counseling and education addressing issues including healthy eating habits, exercise and fitness, stress management, pregnancy prevention, and safer sex
- Provide basic first aid and managing medical emergencies and crisis situations appropriately
- Fulfill other responsibilities that may emerge
Qualifications
- NYS licensed Nurse Practitioner in Psychiatry
- AACN Certification required
- Training and experience working with adolescents and/or young adults with serious mental illness.
- Experience in community-based mental health services.
