Description:

The Clinical Nurse Specialist, in close consultation with PACE primary care, social work, and other members of the PACE Interdisciplinary team (IDT), provides office, home and nursing home-based psychiatric evaluation and treatment for PACE participants. In addition to direct evaluation and treatment, the Clinical Nurse Specialist provides consultation, on request, to members of the IDT.

Essential Duties & Responsibilities:

  • Performs assessments for participants who are referred by PACE primary care or social work.
  • On the basis of evaluation and in consultation with the interdisciplinary team, identifies patients who require ongoing treatment, prepares treatment plans with specific recommendations to primary care staff regarding management of psychotropic and related medications; behavioral or environmental interventions; relevant lab work, testing or other consultation to support appropriate diagnosis and implementation of treatment.
  • Performs follow-up assessment and treatment, in the appropriate setting, for an identified panel of patients who require ongoing management of psychiatric meds and related treatment.
  • Provides limited psychotherapy.
  • Provides appropriate participant and family teaching and education.
  • Provides consultation or direct evaluation in psychiatric emergencies.
  • Provides evaluation for purpose of developing Rogers and other guardianship applications and related treatment plans.

Qualifications and Requirements:

  • Master of Science in Nursing with specialization and board certification in Adult Psychiatric/Mental Health advanced practice nursing.
  • Psychiatric Clinician (PC) licensure in Massachusetts.
  • Prescriptive privileges in Massachusetts
  • Experience providing psychiatric evaluation and treatment for geriatric and medically complex populations with a broad range of mental health, substance abuse and dementia diagnoses.
