On a daily basis, assists Program Supervisor or Scheduling Coordinator and other direct care staff in implementing programs in community settings for individuals served.

High school diploma or general education degree (GED). Good interpersonal and organizational skills. Ability to make sound judgment decisions. Ability to carry out written and oral directions. Must acquire/maintain a Class E Chauffeurs License

The essential duties and responsibilities include the following. Other duties may be assigned.

  • TASKS
    • Assists individuals served in achieving their goals
      • Learns the personalities, habits, routines, and unique communication styles of individuals served and works with them accordingly.
      • Provides encouragement and support to individuals served so they can achieve their desired objectives.
      • Teaches, supports, and models appropriate social skills when working with individuals served; helps individuals served make healthy and safe choices.
      • Encourages and empowers individuals served to express their wants, needs, and individual preferences.
      • Provides training as outlined in the program participant’s Individual Support Plan (ISP).
    • Monitors and assists with daily living activities of individuals served
      • Administers prescriptions and PRN medication as stated on the physician’s orders.
      • Administers medication within the guidelines of being certified in Level I Medication Aide.
      • Responsible for being aware of medication side effects and adverse reactions. Monitors each participant for such side effects and reactions.
      • Provides opportunities for community recreational activities.
      • Assists the program participant by helping build community relationships that will improve the quality of life and enhance the experience of the community. Events and activities should focus on the individuals served’ likes and interests.
      • Assures the safety and well-being of individuals served and co-workers by following CHS policies and obtaining or administering appropriate medical treatment as needed.
      • Assists and/or provides appropriate personal care as needed. This requires repeated bending, lifting, stooping, squatting, and/or reaching.
      • Responsible for following the guidelines set forth in the Financial Procedures and directions given by the House Coordinator/Lead Staff/ Lead Support Specialist and/or the Program Support Assistant while handling money for individuals served and grocery/household money.
    • Completes required documentation accurately
      • Completes the following for each assigned shift:
        • Documentation on ISP objectives – notes progress, changes, concerns, etc. Documents progress on Individual Plan (ISP) objectives daily in a thorough, accurate, and person-centered manner.
        • Medication Administration Records – daily and PRN medications
        • Service notes in CMIS at the time of service delivery
        • Staff Duty checklist
      • Completes Event Reports (ER) accurately and completely within the designated timeframe established by policy.
    • Miscellaneous duties
      • Responsible for knowledge and implementation of agency and departmental policies and procedures. Policies and procedures are accessed through UKG READY.
      • Presents a positive attitude and demeanor when working with individuals served, co-workers, administrative staff, supervisors and/or parents or guardians.
      • Maintains good communication with Program Coordinator, DDP, DDS, or Scheduling Coordinator; by keeping them informed of developments, concerns, and other pertinent issues.
      • Determines when independent, mature judgment decisions are appropriate and when consultation with supervisor is necessary.
      • Accepts and willingly carries out special assignments or duties, when requested.
      • Respects the rights of all individuals served.
      • Projects positive image of CHS and individuals served to the local community.
      • Assures the confidentiality of information regarding individuals served.
      • Ensure adherence to all company policies.
      • Assures expense vouchers are submitted by the assigned deadlines.
      • Utilizes the UKG READY system to clock in and out and transfer time correctly. If errors happen employees are required to utilize the “Time Change Request” option in UKG READY to request corrections to their punch or time-off entries.
      • Advise Community Living RN of individuals served’ medical issues.
      • May be assigned to other locations, counties or in the community as needed based on staffing and individuals served’ needs.
      • Staff will understand and appropriately respond to the unique combination of cultural variables through ongoing emphasis on Cultural Competency and training as defined by the CHS Cultural Competency plan.

______ Activity Center - Groups

  • Actively and efficiently train individuals according to ISP as assigned using special training groups as needed.
  • Complete pertinent documentation as directed in a timely manner including ISP monitoring, daily notes on consumers, 30-day reviews, and quarterly reviews. Documentation or research for ideas is not to be done while providing supports to individuals served. When individuals served are here, they deserve your full attention; be prepared before arrival.

_______ Your assigned documentation period is after individuals served’ departure.

Other times may be necessary with Program Coordinator approval.

  • Prepare ISP materials as required under guidance of Program Coordinator.
  • Provide timely and complete feedback to Program Coordinator on progress and problems relating to the program needs.
  • Aware of needs other staff may have requiring assistance.
  • Reports any medication that is either outdated or needs to be replenished to the Program Coordinator.
  • Transports individuals served, as needed.
  • Schedules time-off in advance, when possible. Time-off is subject to coverage issues and availability.
  • Meals are eaten with the individuals served. A short break (no more than 10 minutes) will be given based on coverage availability and Activity Center Support Specialist agreement.
  • Keeps Program Coordinator or Activity Center Support Specialist informed of changes in behavior or any other factors which may constitute a need for an adjustment in dosage or types of medications.

______ Activity Center – One-on-One

  • Actively and efficiently train individuals according to ISP as assigned.
  • Ensures continuity of support for assigned program participant by maintaining information regarding monthly schedule, general routine sheets, and support and technique sheets in a format easily accessible in your absence.
  • Complete pertinent documentation as directed in a timely manner including ISP monitoring, daily notes on consumers, 30-day reviews, and quarterly reviews. Documentation needs to reflect the custom supports being offered to meet your program participant’s unique needs. Documentation or research for ideas is not to be done while providing supports to program participant. When your individuals served is here, he or she deserves your full attention; be prepared before arrival.

  • Keeps Program Coordinator or Activity Center Support Specialist informed of changes in behavior or any other factors which may constitute a need for an adjustment in dosage or types of medications.
  • Provides assistance to the Activity Center program in the absence of assigned program participant.
  • Ensures program area is tidy and prepared for the next day by disinfecting tables/chairs and equipment is stored properly.

______ Individualized Supported Living (ISL)/Group Home

  • Ensures the preparation of well-balanced meals.
  • Provides transportation as needed, will require the use of personal vehicle.
  • Encourages and teaches individuals served to maintain room and belongings in an orderly manner.
  • Ensures housekeeping tasks are completed while on duty and completes any housekeeping checklists if required.
  • Keeps physician informed of changes in behavior or any other factors which may constitute a need for an adjustment in dosage or types of medications.
  • Billing Sheets (Individuals served/Staff)
  • Works holidays based on program needs or as requested by individuals served.

_______Individualized Supportive Living (ISL)/Group Home Floating 40 Hours

  • Ensures the preparation of well-balanced meals.
  • Provides transportation as needed, will require the use of personal vehicle.
  • Encourages and teaches individuals served to maintain room and belongings in an orderly manner.
  • Ensures housekeeping tasks are completed while on duty and completes any housekeeping checklists if required.
  • Keeps physician informed of changes in behavior or any other factors which may constitute a need for an adjustment in dosage or types of medications.
  • Billing Sheets (Individuals served/Staff)
  • Works holidays based on program needs or as requested by individuals served.
  • Works a core number of hours assigned to a particular Group Home/ISL and the remaining number of hours needed to reach 40 are assigned by the Scheduling Team to meet the needs of individuals served.
