The Non-Medical Case Manager (NMCM) is responsible for providing accurate information and services to those living with HIV. The NMCM works as a part of the statewide Positive Connections team in an effort to end the HIV epidemic. NMCM’s assesses the needs of the client, the client’s family and support system, develop an individualized client care plan, arrange, coordinate, monitor, evaluate and advocate for a comprehensive package of services to meet the specific client’s complex needs. The District Supervisor is responsible for the supervision of the HIV Positive Connections team in his/her assigned district. This position works with the HIV Team, to include Outreach, Prevention, AETC, Part B, C1, C2, and D Coordination. The supervisor will oversee staff in assigned district regarding day-to-day operations and coordinate office coverage. The District Supervisor can be a Non-Medical Case Manager (NMCM) or Medical Case Manager (MCM). Experience within Positive Connections preferred. Travel required approximately 30%.
