Job Description:
Schedule: Monday- Thursday, 10:30am- 9:00pm

The inpatient pharmacist is responsible for reviewing and interpreting providers' orders for all medications, and screening for therapeutic incompatibilities. The pharmacists will assist with the monitoring of drug therapy, with an emphasis on chemotherapy. The pharmacist will be responsible for providing and educating on drug information to physicians, mid-level providers and other clinical staff. The pharmacist will be responsible for oversight of technician staff as it relates to sterile intravenous compounding, including chemotherapy and TPN, medication refilling, and unit dose drug preparations. The pharmacist will participate in quality improvement projects with an emphasis on patient safety. The pharmacist will participate in multi-disciplinary initiatives as requested.

JOB RESPONSIBLITIES

  • Verifies and assesses all medication orders for clinical appropriateness based upon patient's current lab values, symptomology, and disease state(s)/comorbidities.
  • Able to communicate appropriately with providers and other clinical staff regarding medication orders or drug questions.
  • Competent in preparation and dispensing of sterile compounded products following guidelines outlined within USP 797, and USP 800, and able to supervise technician staff performing these functions.
  • Participation in departmental and hospital quality assurance programs with an emphasis on patient safety, and regulatory compliance.
  • Ensures safe, appropriate medication therapies for patients based on established clinical guidelines.
  • Required to participate in clinical documentation for all pharmacist interventions
JOB QUALIFICATIONS

  • Experience - Minimum 2 years institutional pharmacy experience
  • Education - ACPE-accredited School of Pharmacy (RPh or Pharm D)
  • Licensure - License in good standing with Arizona State Board of Pharmacy
