Main Office Location : Londonderry , NH Territory: Salem and Surrounding Areas Job type: Per Diem RN Registered Nurse (RN) Responsibilities:

• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician
• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es)
• Initiates the plan of care and makes necessary revisions as patient status and needs change
• Collaborate with physicians, nurse practitioners and interdisciplinary team to create a strong Plan of Care for each patient
• Respond to inquiries and concerns from patients, their caregivers and their medical team
• Counsel patients and caregivers before, during and after treatment
• Supervise home health aides as needed
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient

• 2+ years’ nursing experience, preferably in geriatrics
• Bachelor’s Degree in Nursing
• Proficient with electronic medical records
• Excellent bedside manner and communication skills
• Working knowledge of health care laws and regulations
