The Turning Point Diversion Program is a “whatever it takes” and “meeting the individual where they are” program. The program is Assertive Community Treatment (ACT) and Full Service Partnership (FSP) model program supporting persons served in reaching their own vision of wellness and recovery. Services are delivered as an outpatient mental health program serving individuals diagnosed with a Serious Mental Illness (SMI), and at risk of being hospitalized, homeless, and/or incarcerated. The program focuses on individual strengths and abilities to successfully gain independence and self-sufficiency in the community. Individuals are assisted with community reentry and empowered to achieve a variety of goals. Services are provided to adults residing in Fresno County. Services can be delivered in the clinic, home, and community based-settings in collaboration with the person served. Services may also be delivered via secured virtual and telehealth applications and by telephone. Services are provided Monday through Friday between 8:00am and 5:00pm. Services are available for crisis and after hour emergencies 24 hours a day, 7 days a week.


Turning Point Diversion Program services provide the full spectrum of community services necessary to attain the goals identified in each person’s treatment plan and Individual Services and Support Plan (ISSP). The program treatment team provides a collaborative relationship between the individual and when appropriate their family members or significant others. The program is comprised of a multidisciplinary team with diverse skills, expertise, and knowledge to help address each individual’s unique needs and challenges. Each person served is assigned a mental health clinician and case manager who act as a care team to support the individual. Other members of the treatment team include licensed vocational nurses, peer support specialist, education and employment specialist, substance use disorder specialist, criminal justice specialist, and a psychiatrist.

JOB SUMMARY

The Peer Support Specialist reports to the Supervising Personal Service Coordinator and serves as a client advocate, provides information and peer support to clients in office outpatient settings, performs a wide range of tasks to assist clients in regaining control over their own lives and over their own recovery process.

ESSENTIAL JOB RESPONSIBILITIES

  • Assist with needs of program treatment staff and clients to provide quality services to Severely Mental Ill (SMI) and dual diagnosis program participants.
  • Remain in full compliance with regulatory and agency policies and procedures, including ensuring confidentiality of verbal and written client information.
  • Be present to meet and greet clients upon arrival to the Program.
  • Provide client with transportation support to and from the clinic as needed. Engage clients in conversations focused on their wellness. Provide activities for clients while they wait for service in the lobby.
  • Provide client support with reviewing the code of conduct and program policies/procedures. Model and encourage socially appropriate behavior and adherence to program guidelines/procedures.
  • Answer the telephone and provide reception support as needed.
  • Provide peer support, life/coping skills, and assistance to clients.
  • Perform data entry, filing and other general office duties as needed
  • Provide supervision and client assistance while in the office setting.
  • Attend daily team check-in meetings, bi-weekly team meetings, and bi-weekly one-on-one meetings with the Assistant Program Director.
  • Will assist with the planning and organization of special socialization groups and events for clients.
  • Work with other team members to achieve client goals and objectives.
  • Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
  • Provide timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
  • Think and act quickly and efficiently in emergencies.
  • Will drive on company business and transport clients as needed.
  • Comply with all Turning Point Safety policies and procedures, including but not limited to: workplace safety, reporting work related injuries, Infection Control and preventing potential safety risks for staff, clients and others.
  • Punctual and regular attendance is an essential responsibility of each employee at Turning Point. Employees are expected to report to work as scheduled, on time and prepared to start working. Employees also are expected to remain at work for their entire work schedule. Late arrival, early departure or other absences from scheduled hours are disruptive and must be avoided.

ESSENTIAL JOB REQUIREMENTS

  • High school diploma or GED.
  • Current or former recipient of client services required.
  • Demonstrated proficiency in reading and writing.
  • Ability to establish trusting relationships with peers. Work effectively with diverse ethnic, Cultural, religious, socioeconomic, and professional groups of people.
  • Ability to work independently and as a part of a team; strong interpersonal and problem Solving skills
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  • Ability to operate within Electronic Medical Records (EMR) System.
  • Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment physical, drug screen, general physical, and TB test.
  • COVID19 vaccination required prior to employment.
  • Must maintain good relationships with clients, co-workers, government and civic representatives, as well as community members within whom the Agency is transacting business, and relate to them in a professional manner.
