Job Summary: The Hospice Peaceful Passing Aide is a paid PRN hospice position working in close connection with the patient, interdisciplinary team, family and facility at a time when increased support and/or interaction with the patient and/or caregiver are deemed appropriate. The Hospice Peaceful Passing Aide may be asked to primarily work at night providing educational, emotional, spiritual, practical and medical support to the patient, family or facilities, including, but not limited to, personal cares, administering medication according to the patient Plan of Care and/or Medication Plan, monitoring patient health status and reporting changes in condition to on-call staff and the care team. The Hospice Peaceful Passing Aide is needed by hospice staff, patient, family and/or the facility. This position also includes providing post-mortem cares and provides supportive measures to complete the death visit. Peaceful Passing Aide is on-call for post-mortem cares, maintaining death protocol and hospice charting. Job Qualifications: Education: High school graduate or have GED. Licensure: Minimum CNA certified. Certified Hospice and Palliative Nurse Assistant (CHPNA) and Peaceful Passing Aide Certified preferred Experience: One year hospice experience Skills: State Medication pass certified and demonstrate ability to properly medicate a non-responsive patient. Possess strong end-of-life knowledge with ability to educate patients, family, facilities, and caregivers about what they are observing. Must possess and demonstrate excellent patient observation, assessment and judgment, and report changes to staff on an ongoing basis. Must be able to provide patient cares, including post-mortem cares and emotional support to patient and families. Provide accurate written documentation and charting in a timely manner (within 24 hours of the visit). Must possess effective communication skills, including active listening skills.
