Employment Type : Full-Time

As an Inpatient BPCI Navigator, you will visit patient rooms to explain the Bundled Payment Care Improvement (BPCI) program and what to expect. You will attend rounds and work closely with the case management team to determine the appropriate next site of care as well as post-discharge needs. You will also help coordinate hand-off to the appropriate outpatient navigator.

The Particulars:

This is a full-time, Mon-Fri position, with 1 day off per pay period. (Total of 72 hours per pay period). No weekends or holidays! You will get to be part of a small, cohesive team and enjoy autonomy in your role. This position also offers the benefit of direct patient interaction without bedside nursing responsibilities.

GENERAL REQUIREMENTS

Education/Experience:

  • Must have bachelor's degree in nursing OR in a health related field. Masters degree in nursing preferred.
  • 3 – 5 years Case Management experience preferred, hospital or community based.

Licensure/Certifications:

  • Must have RN license, as defined by primary work state (Idaho or Oregon), issued prior to hire date.

The Day to Day:

  • Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions.
  • Completes required Remedy U and Episode Connect training.
  • Maintains an accurate in-house census of BPCI patients utilizing Episode Connect.
  • Establishes partnerships with site Experience of Care champions in order to align BPCI efforts with patient satisfactions outcomes.
  • In coordination with hospital based Case Management team, orchestrates in-person or virtual multidisciplinary rounds to gather and document all necessary clinical information from physical therapy, nursing, etc. to determine the best possible plan for the patient.
  • Communicates with each patient's Principle Accountable Practitioner (PAP) to facilitate patient's family meetings, and confirm the discharge plan.
  • In collaboration with the Patient Care Team, identifies all clinical and social concerns of the patients. Ensures documentation in appropriate systems.
  • Coordinates and tracks all patients' discharge dispositions and destinations via communication with the Post-Acute Care (PAC) patient care team.
  • Attends BPCI work group meetings to report on success of program initiatives and processes.
  • Utilizes real-time data and Remedy generated reports to track key performance indicators and identify areas of concern.
  • Must be comfortable operating in a collaborative, shared leadership role.
  • Exceptional interpersonal and relationship building skills required to effectively collaborate with multiple stakeholders to obtain management, staff, and physician involvement and accountability in achieving specific outcomes.
  • Ability to simultaneously address and prioritize multiple projects, demands and issues. Ability to be flexible and adaptable to changing processes and schedule requirements.
  • Must possess excellent verbal and communication skills, presentation skills, advanced analytical skills, be well organized, and maintain a highly professional manner.
  • Demonstrates high degree of self-motivation, assertiveness and goal-attaining capabilities.
  • Effective negotiation, persuasive and business planning skills.
  • Demonstrated ability to acknowledge and analyze formal and informal assessments of health care markets, locally and across the country, and the impact/potential impact on the organization.
  • Possesses knowledge of budget processes, quality improvement, report distribution, The Joint Commission standards, state and federal regulations, six sigma and LEAN methodologies.
  • Proficient in common software applications (e.g., Word, Excel, PowerPoint).
  • Oversees handoff of patient to the post-acute facility, agency and PAP and maintains contact (either phone or in-person) for 90 day episode utilizing a risk assessment to determine when interventions are needed.
