Purpose: The Regional Manager is responsible for assisting the Clinical Director with the development, implementation and evaluation of Grace Healthcare Services hospice care as well as managing all aspects of the interdisciplinary team.

Job Description:

  • Participate in the coordination, planning and delivery of quality and compliant hospice care.
  • Evaluate patient services provided by the interdisciplinary team (IDT).
  • Review care plans and medication lists for new admissions and review charts.
  • Maintain working knowledge of hospice state licensure as well as federal regulations.
  • Monitor services and documentation to meet hospice policies, state and federal regulations.
  • Serve as liaison to Clinical Director regarding challenges and patient care.
  • Supervise and participate in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team including any contracted staff.

Qualifications:

  • NJ Registered Nurse License required.
  • Bachelor’s or associates degree in nursing preferred
  • At least one (1) year experience preferably in hospice or home care, with progressive management responsibilities
  • Excellent communication and interpersonal skills.
