Position Summary

Nurse Practitioner for the Lung Transplant program is responsible for the management of care to a specified group of patients. Duties are a combination of clinical, administrative and teaching responsibilities, as well as providing direct patient care.

Responsibilities

  • Conducts relevant physical exams, documenting pertinent positive and negative findings.
  • Writes prescription(s) according to New York State and federal law.
  • Assesses and documents patient response to treatment and interventions.
  • Identifies a diagnostic impression and/or formulates a differential based on the subjective and objective data obtained.
  • Conducts review of patient records in collaboration with peers and physician colleagues to determine quality and appropriateness of care provided.
  • Formulates a goal directed plan that includes health maintenance, anticipatory guidance and appropriate medical and nursing interventions according to established protocols/practice agreement.
  • Engages in ongoing peer review and utilization review activities, competency evaluations and Medical Board reviews.
  • Obtains and documents subjective history as appropriate to chief complaint or reason for visit.
  • Provides and documents health teaching that is oriented to the appropriate problem and/or developmentally age-appropriate health maintenance or anticipatory guidance issues.
  • Generates an evaluation summary report for presentation at the multidisciplinary selection meetings.
  • Participates and presents at patient educational seminars.
  • Performs discharge teaching when needed.
  • Performs additional relate duties as needed.

Minimum Qualifications

  • Master’s degree or equivalent in education and experience, plus one to two years of experience
  • Current NY State NP or PA license

Preferred Qualifications

  • Experience in coordination of care of transplant patients

Other Requirements

  • Participation in the medical surveillance program
  • Contact with patients and/or human research subjects
  • Potential bloodborne pathogen exposure
  • Successful completion of applicable compliance and systems training requirements
