Rounds on psychiatric inpatients on the Behavioral Health Unit Initiates the multidisciplinary teaching plan which provides information to patient/ family/ significant other in the areas of (as appropriate): the safe and effective use of medications, nutrition/modified diets, potential food/drug interactions, safe and effective use of equipment, rehabilitative techniques, access to community resources and when and how to obtain further treatment. Communicates the plan of care across the continuum by thoroughly completing appropriate transfer forms and discharge summary forms. Selects therapeutic interventions which are appropriate to the patient’s needs and desired outcomes. Implements care according to the patient’s plan of care in collaboration with the patient/significant other and health care team. Exercises critical thinking skills to make sound clinical judgments about patient care as evidenced by practice patterns, documentation and patient outcomes. Uses appropriate verbal and non-verbal communication to demonstrate warmth and caring and to provide a healing environment. Maintains competency in professional nursing practice and additional competencies as appropriate to the patient population served. Recognizes life-threatening situations and intervenes correctly with minimal supervision. Reports changes in patient condition in a timely and effective manner. Documents the patient’s condition and capability to manage continuing care needs in the discharge/transfer note. One-year experience as an APP preferred but not required Current licensure to practice professional Nursing in state of Texas or in a qualifying compact State. Registered as advanced Nurse Practitioner, Certified in Basic Life Support. Job Summary: The Advanced Practitioner will be assigned to carry out functions of care with the focus on the inpatient psychiatric population as deemed appropriate by his/her supervising physician and in the strict accordance with the established protocols and procedures. The Advanced Practitioner is a professional mid-level provider licensed in Texas to function in an expanded role to provide psychiatric care to individuals, families or groups. The position functions in a collegial relationship with physicians and other health professionals in the delivery of health care services. Provides care for patients of the practice within the scope of licensure. They are able to make independent decisions with physicians in carrying out health regimens for families and clients with psychiatric diagnosis. The Advanced Practice Provider demonstrates ICARE, is proficient in CPOE and is a resource to the physicians, nurses and other disciplines. In collaboration with the physician, the Advanced Practitioner is able to perform a comprehensive History and Physical, develop and implement a plan of care, order and interpret results of laboratory and diagnostic tests, provide psychiatric care to include, coordination and initiation of consults, review and adjust medications and dosages, provide patient education and comprehensive discharge planning. The position consults and collaborates with other physicians and disciplines to ensure smooth and timely throughput, including coordination of discharges and transfers. The Advanced Practitioner documents and meets criteria for evidence based and quality outcomes. The Advanced Practitioner may work closely with utilization review, multidisciplinary teams and floor managers to achieve prompt and proper admission and placement, optimize length of stay, and provides superior quality service.
