Palomar Heights Post Acute Rehab is looking for an LVN Case Manager to join our team.
Purpose: To work passionately and holistically to ensure that the care and discharge plans meet the physical, social, and emotional needs of our patients.
Duties and Responsibilities:
- Coordinate patient care and services.
- Coordinate, monitor, and evaluate services according to the needs of patient.
- Work together with the patient and family, care team, payers, and external entities to promote a safe transition from the facility to the next provider or care setting.
- Work to promote a safe transition from the facility to the next provider or care setting in accordance with all applicable laws, regulations, and Life Care standards.
- Make independent decisions when circumstances warrant such action
- Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
- Implement and interpret the programs, goals, objectives, policies, and procedures of the department
- Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
Requirements and Qualifications:
- Currently licensed/registered in applicable State.
- Must maintain an active LPN/LVN license in good standing throughout employment.
- One (1) year of clinical experience in post acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Generate written communication that is clear, concise, and well organized
- Excellent organizational skills and be efficient in prioritizing and managing time and assignments
- Maintain professional working relationships with all associates, vendors, etc.
- Maintain confidentiality of all proprietary and/or confidential information
- Understand and follow company policies including harassment and compliance procedures
- Displays integrity and professionalism by adhering to Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training
Physical Capacities: (With or Without the Aid of Mechanical Devices)
- Must be able to move intermittently throughout the workday.
- Possible bending, squatting, twisting and reaching
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met
- Must meet the general health requirements set forth by the policies of this facility
- Must be able to stand for and walk for extended periods of time
- Must be able to cope with the mental and emotional stress of the position
Environmental Conditions: Inside work, normal temperatures, some noise, occasional fumes/odors, potential exposure to bodily fluids.
