Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We are looking for a Navigator II for our H3 Veterans program at our Central Phoenix location.

A Navigator H3 is an individual who is in recovery from AOD use and/or GMH for an appropriate period of time (greater than or equal to 1 year) and whose primary role is working with Project H3 patients as their Navigator for housing, services and ongoing support. The Navigator H3 serves as the bridge for the patient, in coordinating and monitoring RHBA and Non-RBHA community based services and facilitates delivery of these services by supporting the individual. The Navigator H3 will work with individuals who have been determined as the most vulnerable homeless according to the vulnerability index and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator H3 will also map out the most appropriate next steps for the individual who may need assistance in accessing various services after or before the individual has been housed. The Navigator H3 works in conjunction as part of a multi-disciplinary team made up of other Community Agencies and stakeholders, CBI PSS, CBI Peer Services & Outreach Assistant Manager and/or Manager. A Navigator H3 will document and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Polices and Procedures, and specific for the H3 Program. The Navigator H3 will also carry out other duties assigned by immediate supervisor and/or clinical supervisor.

Skills and Requirements

  • Must have one of the following:
    • Master's degree or bachelor's degree in a field related to behavioral health, is a registered nurse, is a physician assistant who is not working as a medical practitioner
    • Bachelor's degree and at least one year of full-time behavioral health work experience
    • Associate's degree and at least two years of full-time behavioral health work experience
    • High school diploma or high school equivalency diploma and 18 credit hours of post-high school education in a field related to behavioral health and two year of full time behavioral health work experience or four year of full time behavioral health work experience or is licensed as a practical nurse with two years of full-time behavioral health work experience.
  • Requires a valid Arizona Driver’s License and 39 month clear MVR

  • The Navigator is an individual who is in recovery from substance use and/or GMH for an appropriate period of time (greater than or equal to one year)
  • Candidates must be a minimum of 21 years of age to meet the minimum age requirement to work for CBI
