Job Summary[A3]
This role is an expert resource to the care team, physicians, patients, and families, by assessing, planning, and facilitating social and clinical services to meet member’s needs. The role will utilize advanced clinical judgement and critical thinking skills to perform care management duties, such as monitoring members with co-morbidities and other social detriments to their health to assure the receive the best possible care. This role will work with a number of key stakeholders; BTP primary care providers, and specialists, as well as hospital, skilled nursing, and health plan partners.

Assists care team, providers, and physicians in assessing and meeting the needs of patients and families experiencing complex biomedical and psychosocial needs
