Middlesex Rapid-Rehousing Case Manager

Purpose

Under the direction of the Manager of Middlesex Services, the MX Rapid Re-housing Case Manager assists people with the RRH application process to provide on-going support to eligible applicants with case management which offers a structured move to housing. The RRH Case Manager provides case management services to help individuals in finding, securing and maintaining housing and will provide linkages to educational, employment, vocational, spiritual and recreational opportunities in addition to mental health, substance abuse and medical treatment.

Qualifications, Knowledge, Skills and Abilities

Bachelor’s Degree in Human Services required with a minimum of two years’ experience working in human services or related fields. Excellent organizational and time management skills with a deadline-oriented mindset. Strong written and oral communication skills employing the English language. Proficient is Microsoft Office and other common desk top applications. Able to effectively use the local network of substance abuse, mental health, and social services providers. Familiar with criminal justice and able to engage, manage, motivate, and support. Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics. Complete knowledge base of all aspects of the delivery of person-centered case management. Able to meet all documentation requirements. Knowledgeable of crisis intervention, boundaries, confidentiality, universal precautions, harm reduction. Knowledge in housing first, housing law and tenant rights. Familiar with landlords in the greater New Haven area. Valid drivers’ license and a good driving record.

Duties and Responsibilities

Provide linkages for clients who are in need of social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services. Ensure that those who need mainstream benefit programs (e.g., SSI/SSDI, TANF, SNAP, etc.) are referred to the SOAR worker for eligibility screening. Continuously prepare the client for transfer of care to permanent housing and recovery support caregivers. Coordinate and collaborate with other agencies and social service providers. Develop mutually reinforcing relationships with landlords and assist residents with finding housing as needed. Attend weekly and monthly agency, program and network meetings. Monitor all cases assigned and complete all documentation/charting as required. Create strength-based person- centered service plans. Other related duties as assigned

Salary Range: $17.30-$19.23/hr. based on experience plus excellent benefits

Schedule: Monday through Friday 9:00 am to 5:00 pm, flexibility required
