As a Medical Records Clerk, you will be responsible for maintain, incorporating and accurately accounting for all medical records information on each individual resident. Also, ensure that the confidentiality and compliance with HIPAA and state and federal regulations.
Day-to-Day Responsibilities:
- Maintain daily resident roster to match billing access.
- Ensure all medications have the appropriate diagnosis.
- Use ICD-9 coding for each diagnosis and entered in to the computer.
- Ensures that physician’s orders, telephone orders, and other records requiring signatures are received and filed within the required time frame.
- Ensure that all residents are seen by their physicians and routine lab work is completed and filed in the required time frame.
- Keep resident register and assign medical record number.
- Help transport residents and answers telephones and files.
General Responsibilities:
- File resident medical information in the resident record in a chronological way.
- Assures that each section of the resident’s record is maintained in a neat, organized, and readable format.
- Prepare lab requisitions according to physician’s orders.
- Pull lab results from computer/fax and report to charge nurse.
- Prepare face sheet for new admissions and update as necessary.
Qualifications:
- Current Certified Health Information Specialist or a State Licensed Practical Nurse License.
Working Conditions:
- Long-term care, skilled nursing, or assisted living medical environment.
- May require standing and walking for up to 50% of the time.
- Work is generally performed within an office environment with standard office equipment available.
- The employee is required to use assistance when attempting to assist, lift, or care objects over 25 pounds.
