MDS COORDINATOR Details

As a registered nurse, or under the direction of a registered nurse, this position is responsible for the development, coordination, and completion of MDS assessments for the Veterans/Members of an Arizona State Veteran Home, a skilled nursing facility. This includes development of procedures, maintenance of the MDS assessment, and development and scheduling of the care plan. Ensures the accuracy of captured information and timely transfer of data to required agencies in compliance with federal, state, and VA regulations. Incorporates a cultural focus on Veteran/Member-centered care in the performance of job duties. Learns, implements and monitors Arizona Management System AMS methods. Utilizes AMS concepts and tools for problem solving and continuous work/process improvements and efficiencies.

Job Duties:
  • Coordinates the development and implementation of the Veteran/Member MDS. Ensures all aspects of the prospective payment system comply with federal rules e.g., False Claims Act , state regulations and Medicare guidelines governing the MDS. Maintains and updates written policies and procedures.
  • Monitors completion of supportive documentation and MDS by the interdisciplinary team for all Veterans/ Members. Performs follow-up assessments to ensure the appropriate care is provided and documented for appropriate levels of reimbursement.
  • Tracks and completes all MDS assessments. Ensures prompt entering of data into clinical software. Verifies data, timely date stamp without errors, and provides notification of issues related to payment or timeliness to the Administrator. Ensures appropriate Medicare notice to Veterans/Members.
  • Coordinates and schedules weekly care plan lists per the regulations. Informs team of updates and schedules.
  • Monitors Veterans/Members for significant changes of condition and initiates new MDS when needed.
  • Educates the multidisciplinary team on updates related to MDS, prospective payment system or other related regulatory changes.
  • Serves as primary contact with the software provider that supplies support for the MDS document to address problems related to the completion and transmission of data in a timely manner. Serves as primary contact with state representative who manage MDS submissions.
  • Serves on committees, attends meetings, and participates in special projects assigned by the Administrator. Attends in-service training. Actively involved in AMS processes such as Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work. Participates fully in the QAPI process, the
Quality Assurance and Performance Improvement program mandate by CMS, which incorporates AMS.
Knowledge, Skills & Abilities (KSAs): Knowledge of:
  • Knowledge of nursing processes, practices and techniques
  • Strong working knowledge of current state and federal regulations regarding Minimum Data Set, resident care plans, quality of care measurements and standards of care for skilled nursing facilities
  • Knowledge of OBRA and state operational manual that dictates how and when MDS is completed and how assessment is applied to functional care plans
  • Knowledge of Medicare prospective payment system in relation to the MDS and residents' clinical care needs
  • Knowledge of computer software that is used for completion and transmission of MDS

Pre-Employment Requirements:
  • Must have a current license to practice as a Registered Nurse or Licensed Practical Nurse in Arizona.
  • Must be able to obtain and retain a valid AZ DPS Fingerprint Clearance Card.
