REPORTS TO: Director of Nursing/Nurse Manager
JOB OVERVIEW: Provides direct and indirect care in the inpatient setting. Communicates with physicians, DON, Therapy Dept., and co-workers, as appropriate about changes in patient’s clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties with adequate supervision.
ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:
a. Knowledgeable of medications and their correct administration based on age of the patient and their clinical condition.
b. Follows the five medication rights and reduces the potential for medication errors.
c. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
d. Able to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes adult, geriatric and the general patient population.
e. Able to revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness.
f. Consults with therapy staff daily to provide for an interdisciplinary approach to the patient’s needs and documents patient’s status to include rehabilitation therapy progress.
g. Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills.
h. Able to adequately assist and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
i. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
j. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
k. Demonstrates ability to perform treatments and provide services to level of licensure.
l. Communicates appropriately and clearly to DON, coworkers, physicians and the Therapy Department staff.
m. Integrates the nursing plan of care with the Therapy Department treatment plan to attain the patient’s optimal level of functioning.
n. Participates in patient care conferences.
o. Provides care appropriate to condition and age of the patient, including adult, geriatric and general population.
p. Obtain intravenous access as needed and administer intravenous medications according to scope of practice.
q. Perform veinapuncture according to order to assist physicians with care and diagnosis.
r. Perform skin/wound assessment every shift. Provides wound care as ordered by physician or other licensed provider.
s. Demonstrates and ability to be effective, flexible, well organized and function well under stressful situations, to provide proper patient care.

w. Treats patients and their families with respect and dignity; ensures patient confidentiality.
x. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
y. Maintains a good working relationship both within the department and with other departments
z. Maintains stock supply levels on unit. Notifies charge nurse or DON when supplies are low
aa. Ensures QC is completed: Check and record refrigerator temperature checks, CBG checks and eye wash station.
bb. Orders medication from the pharmacy ensuring that OTC medications are not ordered and an adequate amount of PRN medications are available.
cc. Verifies all pharmacy deliveries. Refuses delivery of medication for discharged patients or discontinued medication.
dd. Documentation meets current standards and policies.
ee. Identifies and addresses psychosocial needs of patients and family; communicates with Social Services/Discharge Planner regarding both in hospital and post discharge needs.
ff. Manages and operates equipment safely and correctly.
gg. Coordinates and supervises patient care as necessary.
hh. Skills Checklists/Competencies submitted timely.
ii. Follows United Medical Hospitals' Corporate Compliance Code of Conduct as evidenced through practice, performance, and attendance of annual review programs/in-services.
jj. Performs other duties as assigned or directed to ensure smooth operation of the organization.
