We are seeking a LPN, Non Contract - Home Health East Portland that will work under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assist with collection of assessment data and provides input to the planning of care. Implements plan of care within the scope of licensure. Perform all duties in a manner which promotes team concept and reflects the Providence mission and philosophy.

In this position you will have the following responsibilities:

  • Provide patient care to patients in their homes
  • Collaborate with team members to deliver care and document observations
  • Contribute to patient assessment under the supervision of a registered nurse by assisting in identifying changes in patient status
  • Utilize appropriate standards of care for the identified medical diagnosis
  • Perform basic patient teaching in the home situation according to the plan of care
  • Utilize effective communications skills with patients and their families as well as the home health/hospice team members
  • Utilize Universal Precautions and safety measures to ensure a safe care environment for patients
  • Perform procedures and treatments according to accepted agency practices
  • Demonstrate competencies and knowledge related in the standards of practice for a licensed practical nurse in the state of Oregon when providing care
  • Demonstrate an understanding and respect for patients rights when providing care
  • Utilize positive problem solving when addressing care issues
  • Demonstrate flexibility in meeting agency requirements
  • Actively support and incorporates the mission and core values into daily activities
  • Treat all others with respect and demonstrates excellence, justice and compassion in daily work and relationships with others
  • Demonstrate competency by maintaining positive, constructive interpersonal relationships, and by understanding and practicing the principles of effective teamwork
  • Protect confidential/private information related to patients, members, employees, and others
  • Consistently demonstrate and incorporate principles of safety and infection control into daily activities and outlined in Environment of Care, Infection Control, and Exposure Control manuals and department safety policies/procedures
  • Consistently uses personal protective equipment as required and takes appropriate precautions whenever there is potential for contact with blood, body fluids, chemicals and/or other hazardous materials
  • Maintain knowledge of work-appropriate aspects of environment of care programs, complies with policies and reports unsafe conditions
  • Successfully complete Environment of Care healthstream modules in the required timeframes and participates in fire drills and emergency exercises, may label, transport and store hazardous waste as described in annual training
  • Demonstrate performance by adhering to established policies and procedure and exhibiting the defined characteristics associated with attendance and punctuality
  • Demonstrate competency by knowing and applying the principles of an inclusive work environment

Qualifications:

Required qualifications for this position includes:

  • Graduate of an accredited Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) school of nursing
  • Current Oregon Licensed Practical Nurse (LPN) license
  • Cardiopulmonary resuscitation (CPR) certification
  • One (1) year of experience in nursing; long term care, acute care, ambulatory care or home health and hospice care
  • Valid drivers license and reliable transportation
  • Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
  • Current AHA-BLS (Basic Life Support for Health Care Provider or ACLS) within thirty (30) days of hire
  • In compliance with the Providence St. Joseph Health - OR, criminal background check

Preferred qualifications for this position include:

  • Experience in home health, hospice or public health
