Looking for a qualified Licenced Practical Nurse to join a management team passionately committed to providing the highest level of care and lifestyle quality to residents in an assisted living community.
Summary of Responsibilities:
- Assessment of individuals for admission to and discharge from the Promenade and ALP
- Oversee the Resident Care Coordinator, LPN Med Manager responsibilities and tasks
- Completes and assesses an initial, 6 months interim assessment, change in condition and annual UAS evaluations, ISP’s, and ongoing case management progress notes
- Follows physician directions on development of plan of care for residents
- Creates and Reviews the Care Plan for all new admissions and as necessary for AL and ALP residents
- Coordinates medical appointments as needed and corresponds with resident physician offices
- Helps in attaining annual medical evaluations
- Maintains the confidentiality of all protected health information as set forth by the HIPAA Privacy Rule
- Ensure that the Medication Administration Policy and Procedures is followed at all times
- Makes initial and follow-up visits to hospitals, rehabs and SNF’s and documents in chart
- Attends all mandatory facility in services
- Ensures appropriate and detailed documentation is maintained for resident files, DOH reporting requirements and staff records.
- Identifies needed policy changes or development regarding ALP services
- Demonstrates a thorough knowledge of all County, State and Federal regulations related to Adult home and ALP services
- Provides data and information with respect to NYS assisted living reporting requirements
- Maintain a continuous collaboration with residents and family members that conveys the company’s utmost commitment to the care, well-being and quality of life of its residents
- Provide direction and guidance to wellness staff, including hiring and training as appropriate
- Directs and maintains infection prevention and control protocols within the facility
- On-call 24/7 hours for Emergencies and Manager on Duty responsibilities
- Other duties as may be required or necessary
Qualifications:
- NYS Licensed and currently registered as an LPN
- Experience in a patient care setting, acute care desirable
- Previous experience in NYS ACF, ALP, and/or LHCSA desireable
- Excellent clinical, writing and communication skills
- Familiar with NYS DOH regulations
- Must be able to lift 25-35 pounds
