Social Worker Needed-Hospital Position
Full Time Temp Position M-F

A major hospital in Brooklyn is looking for a social worker for a full time temporary position for discharge planning and other services as needed. This position is from 9/7/2021-10/12/2021

Job Description:

1. Ideal candidate should be outcome driven with outstanding interpersonal skills and ability to cultivate effective relationships in an interdisciplinary, fast-paced environment.

2. Ability to excel in fast-paced and outcome driven environment preferred.

2. The ability to communicate and work collaboratively in a multidisciplinary setting required.

3. Flexibility, compassion and ability to work independently are also desired.

4. Key responsibilities includes Discharge Planning, Biopsychosocial Assessment, use of Community Resources and Care Coordination. .

Requirements/Qualifications:

1. Must possess LMSW by New York State.

2. Discharge planning experience in medical acute care setting is preferred including inpatient and outpatient social work.

3. A solid knowledge of community based referral sources.

4. Excellent organizational, critical and independent thinking skills required.
