SUMMARY
The Lifestyle Assistant/Driver conducts social, religious, educational and service-related programs for the assisted living residents as directed by the Lifestyle Director. Said activities are experienced both within the community and in the community as a whole.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Prepares and conducts scheduled group activities.
  • Transport residents to and from activities.
  • Perform activities with room bound residents.
  • Record daily residents’ attendance at group activities and documents room bound residents’ responses to activities.
  • Monitor grooming of residents and assists if necessary.
  • Deliver mail to residents and reads mail to those who needs assistance.
  • Assists residents with menu selections where applicable.
  • Treat residents with dignity and observes all residents’ rights and confidentiality.
  • Communicate with staff in other departments regarding any changes in resident’s condition and regarding information about scheduled activities.
  • Covers various shifts at the Front Desk as specified on Work Schedule.
  • Assists Lifestyle Director with office work and program planning.
  • Hosts and oversees smooth operation of large group activities as scheduled.
  • Willing to support residents in finding activities of their choosing and in daily problem solving.
  • Shops for activities if needed.
  • Arranges furniture for specific programs if needed.
  • Assures safety of passengers and company vehicles.
  • Drives company van or bus for doctor appointments, shopping and special events within company transportation schedule.
  • Attends to special personal needs of passengers when necessary without compromising first priority duties (i.e. safety and driving).
  • Maintains vehicle maintenance and documents daily the condition of the vehicle(s) in logbook in accordance with Sonata Senior Living requirements.
  • Maintains friendly and concerned relationship with residents while upholding the Company’s policies and procedures.

  • Supports a dignified and caring atmosphere with residents, residents' families, visitors and team members.
  • Protects privacy and confidentiality of information pertaining to the resident, team member, residence, company information and records.
  • Maintains a safe and secure working environment and practices safe working habits.
  • Complies with the attendance policy.
  • Complies with the cell phone and social media policies.
  • Maintains neat appearance, good personal hygiene and appropriate attire per dress code.
  • Participates in training, in services and attends meetings when required.
  • Assists with special projects as assigned and performs other related duties.

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Prior to presence in the community, applicant must have completed a Post Hire Health Questionnaire upon offer of employment. A TB test result must be received with 7 days of employment. Previous TB test results cannot be older than 6 months; chest x-ray results 2 years. A Criminal Record Clearance is required prior to team member’s starting date.

If also working as the Transportation Driver, the following qualifications must be met: Must be at least 18 years of age and be in possession of a current State Driver's License and a Class "B" driver's license for transporting groups of people. Must have one year or more of public transportation experience. Must have a clear DMV record and be enrolled in the DMV Pull Notice program.

EDUCATION and/or EXPERIENCE
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Must be at least 18 years of age and have a minimum of two years experience with geriatric residents or two years experience in the human services area. Must have demonstrated social and communication skills, both written and verbal.

LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or team members of organization.

CERTIFICATES, LICENSES, REGISTRATIONS
Most possess a valid driver’s license and have access to a private vehicle for business use. Must carry and maintain current first aide certificate. May require a CDL license.

PHYSICAL DEMANDSPosition requires the ability to stand, reach, bend/stoop/crouch, push/pull/lift/carry up to 50lbs, twisting/ turning and repetitive motion

PHYSICAL ABILITIESMust have good vision, hearing and speech
