SUMMARY
This position under supervision provides comprehensive assessment, case management, consultation and treatment for a wide range of behavioral health issues in a community health center setting. Supports behavioral health care provided by primary care providers, in consultation with a psychiatric specialist and or other clinical staff as appropriate. Coordinate referrals to clinically indicated services outside the primary care clinic (e.g. social services, behavioral health specialty care, substance abuse treatment, and the like) improve the operations of PCHS in support of its stated mission: providing accessible, affordable, quality health and wellness services for our communities.*

Essential Duties and Responsibilities Under Supervision include the following. Other duties may be assigned.
Support and closely coordinate mental health care with the patient’s primary care provider and other treating mental health providers.
Screen and assess patients for common mental and substance abuse disorders.
Provide patient education about common mental and substance abuse disorders and available treatment options.
Monitor clients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management prescribed by primary care provider, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
Provide brief counseling or psychotherapy using evidence-based techniques such as behavioral activation, motivational interviewing, or other relevant skills.
Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
Conduct regularly scheduled (usually weekly) caseload consultation with program director. Communicating resulting treatment recommendations to the patient’s Primary Care Provider. These consultations will primarily focus on patients who are new to treatment or who are not improving as expected.
Facilitate patient engagement and follow-up in mental health care.
Track patient follow-up and clinical outcomes using the web-based care management tracking system (MHITS). Document in person and telephone encounters in MHITS and use the system to identify and reengage patients who may be lost to follow up.
Document patient progress and treatment recommendations in MHITS, in order to facilitate information sharing with Primary Care Providers and other treatment providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the Primary Care Providers and psychiatric specialist. These may include changes in medications or psychosocial treatments or appropriate referrals for additional services.
Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Facilitate Social Security Disability eligibility process with community service office.
Complete relapse prevention plans with clients who are in remission.
Participate in regular training and technical assistance activities for Mental Health Coordinators.

Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
Education and/or Experience
Master’s Degree in Clinical Social Work or Mental Health Counseling, minimum of two years direct clinical experience required. Experienced working with screening tools, evidence based counseling techniques, brief treatment (e.g. CBT, PST-PC), crisis intervention, complex and chronic mental illness, and substance abuse. Knowledge of psychopharmacology, GA-U, FA-X and SSI program requirements.

Certificates, Licenses, Registrations
The following is required for this position:

  • Seven hours of HIV/AIDS Training
  • CPR Certification (CPR Certification will be renewed on an annual basis)
  • Professional license from Washington State Department of Health (such as; Clinical Social Worker License, Mental Health Counsel License, Social Worker Independent Clinical License, Licensed Marriage and Family Therapist)
  • Valid driver's license
  • Automobile insurance.

All certification & licenses must be kept in a current or renewed manner at all times. No inactive, expired certifications or license are allowed.
