SUMMARY
The Wellness Nurse is responsible for assisting with resident care in order to ensure the residents remain as independent as possible for as long as possible. Assist the resident and his or her family in maintaining the physical and emotional health of the residents. Liaison with hospital personnel, physicians, community organizations and other health related service agencies to provide care to the residents. This position is responsible for the care needs in the Traditional Assisted Living and Memory Care Center sections within the community. Coordinate, motivate, monitor performance, schedule and supervise department staff in accordance with company policy. Make recommendations to the Wellness Director with respect to staff evaluations, reprimands, and disciplinary actions.

Ensures provision of services to the residents and ensures that the Federal and State policies governing resident care are closely monitored and carried out.
Assist in the scheduling and supervise department staff in accordance with the company policy. Recruitment and selection should focus on qualified candidates with interest in care and management of individuals with dementia. Conduct with the Wellness Director with respect to reprimands, disciplinary actions and terminations.
Assists with departmental schedule and ensure adequate staffing in accordance with company standards, policies, procedures, budgets and state law.
Assists with continuing education programs for nurses, medication technicians, and caregivers as regulated by Federal and State licensing requirements and assist the Business Office for tracking orientation/training/license requirements.
Assists with orientation for new members of the department to ensure that employees are knowledgeable of the goals, policies and procedures for the department.
Assist and provide necessary training to department staff that ensures competent delivery of medications to residents, emergency procedures, record keeping, residents’ records and acceptable treatments. Assist the Wellness Director for care staff (AL and/or MC) to complete state required training.
Arrange for coverage of job duties within the department during employee absences either through delegation or personal completion of duties.
Conduct and coordinate the completion of resident assessment/evaluation program in accordance with current rules, regulations, guidelines and company policies that govern resident assessment; administer pre-admission and periodic assessments to residents and monitor changes in status. Ensure that an initial Resident Assessment/Evaluation is completed prior to each resident’s move-in date and an individualized service plan is completed upon move-in within the required timeframe. Provide reassessments/re-evaluations on a regular basis.
Conduct interviews and observations on a regular basis of the residents to alert the Wellness Director, Executive Director, family, physician and others of changes in the physical or emotional health of the resident.
Complete required data entry into EMR system to ensure maintenance of resident care information. Ensure that Individual Service Plan and assignment sheets are completed timely and required documentation has been completed.
Develop methods for coordination of nursing services with other resident services to ensure the continuity of the residents’ total regimen of care.
Act as care manager and resource person for the residents and their families. Contact physicians, family and others to obtain specific orders for care and represent the property to professional personnel, residents and the community.
Coordinate orders with physicians/authorized prescribers and pharmacy for delivery of medications. Assist with the oversight of central storage and delivery of medications.
Communicate with residents, family members regarding the property’s in-house pharmacy and the policies/procedures governing that relationship.
Conduct property tours and promote the property to members of the community.
Supports the community’s success in referral development and resident retention efforts to assist with maintaining high census.
Participate in property surveys (inspections) made by authorized governmental agencies.
Maintain a safe and secure environment for staff, residents and guests, following established safety standards. Responsible for coordinating special provisions for the safety of residents with special needs, i.e., visual deficits, hearing loss.
Perform fire and safety procedures when needed to assure resident/staff safety and coordinate with maintenance/environmental services for life safety procedures.
Maintain inventory of needed supplies.
Provide the Wellness Director with information relative to the care needs of the residents and the department’s ability to meet those needs.
Participate in daily and monthly meetings to communicate key issues within the department and care staff.
Assist in monitoring department costs on a day-to-day basis.
Maintain knowledge in dementia-specific and memory care. Recognize and respond to changes in the dementia care industry and respond appropriately.
Performs day-to-day clerical work connected with the position.
Assist in the maintenance of the listing of location of oxygen tanks in the community and provide the current list for the use in emergencies.
Promotes and displays a spirit of teamwork in performance of daily duties through cooperative interactions with co-workers and other departments.
Support a positive and professional image through actions and dress.
Performs other duties consistent with the position as assigned by the Wellness Director.
Maintains Nursing CEU’s as required by the Board of Nurse Examiners and keep license active and in good standing.
Report occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the property’s policies and procedures governing accidents and incidents.
Work directly with residents, providing them with basic essential care including taking blood pressure, recording vital signs, assisting with Activities of Daily Living and Independent Activities of Daily Living.
Obtain resident specific care information from care staff and care plan, provide observations, and report information to health care providers within the nursing care plan in accordance with state guidelines and board of nursing rules and regulations.
Observe and report information that is necessary for care plan development and individual service plans in accordance with state guidelines and board of nursing rules and regulations.
Provide care related teaching to residents and staff within state guidelines and board of nursing rules and regulations.
Work closely with both patients and other health care providers to promote individualized care plans.
Observe and report diagnostic tests in accordance with board of nursing regulations, policies, and regulations to physicians and other health care providers.
Act as a clinical mentor to other LPNs/LVNs and CNAs/Medication Technicians.
MINIMUM QUALIFICATIONS
Education: LVN/LPN or two (2) years of college in human services or health care field. License must be free of stipulations.
Experience: Prefer two (2) years’ experience providing residential care to the elderly or similar experience with specialized dementia care programs.
Mathematical Skills: Ability to perform calculations in support of personnel actions, budget, and other financial responsibilities.
Reasoning Skills: Understand and conceptualize concepts as well as demonstrate ability to make decisions in stressful situations.
Oral/Written Communication Skills: Be able to explain ideas and to communicate complex ideas to a wide audience (employees, residents, family members, governmental agencies, public, etc.), both in writing and verbally.
Equipment Used: Be proficient in basic office equipment (including a personal computer and handheld devices) for the purpose of accomplishing and maintaining a high level of job performance.
Physical Effort: Must be mobile and able to perform physical requirements of the job. Good physical health verified by a health screening, including a chest x-ray or an intradermal Mantoux test not more than 6 months prior or seven (7) days after employment as required.
