GENERAL STATEMENT OF DUTIES

Under limited direction and partnership with Administrator(s) and Registered Nurse, is responsible for the oversight, coordination, and management of nursing, clinical, and medication services, as well as related services that impact the physical, psychological, emotional, and overall resident well-being within an assigned geographic region; teaching, training, and delegating clinical processes and other pertinent healthcare information to team members; and compliance with all nursing and assisted living regulations. Performs related duties as required.

ESSENTIAL FUNCTIONS

• Observes, records, and reports the general physical and mental condition of clients to the registered nurse and/or Administrator;
• Delegates assistance of resident medications to Medication Technicians and/or Caregivers under registered nurse supervision;
• Notifies registered nurse, Administrator, and physician if the clients condition or needs change;
• Performs nursing procedures and treatments according to the needs of the resident and as indicated on the written plan of care and physician orders;
• Administers prescribed medication and treatments according to law, standard nursing practice, and as permitted within the scope of licensure and per physician orders;
• Documents clinical notes for the clients chart for each visit or contact;
• Provides education and care to the client and/or caregiving staff according to the plan of care;
• Assists the Registered Nurse in performing specialized procedures;
• Performs on-call duties as assigned;
• Communicates client needs to the Administrator and registered nurse;
• Coordinates client care needs with outside agencies;
• Assists in the development of the plan of care;
• Assists in development of behavior plans;
• Assists in the management of resident files;
• Performs evaluations of potential residents.

ADDITIONAL JOB DUTIES:

• Performs post mortem care as needed;
• Acts as a liaison in the community for Ashley Manor with the intent of educating individuals about the company and services offered;
• Performs related duties as required.

JOB SPECIFICATIONS:

• Licensed Practical Nurse (LPN) with a current license to practice in the state of operation;
• Minimum of one (1) year of geriatric/senior living nursing experience preferred;
• Minimum of one (1) year supervisory and management experience preferred;
• Certifications in First Aid, Cardiopulmonary Resuscitation (CPR), and medications required;

Knowledge of state rules, regulations, fire codes and other safety practices;

Knowledge of all residents rights, including the right to be free of restraints and abuse;

• Ability to communicate effectively verbally and in writing;
• Ability to use office equipment such as computers, printers, fax machines, email;

Ability to maintain confidentiality of employee and facility issues and records.

OTHER REQUIREMENTS

Complete continuing education required by the state licensing agency;

Schedule flexibility and local/regional travel required;

Must pass a criminal background check;

Must possess valid drivers license, reliable transportation, and personal automobile insurance;

WORK ENVIRONMENT AND PHYSICAL DEMANDS:

• Work is performed both in an office and a health services environment and requires sufficient mobility and physical reflexes to perform in both environments;
• Must be able to lift up to thirty-five (35) pounds unassisted;

Local/regional travel required; may be required to travel for meetings, trainings, or to support other facilities;

May be required to work irregular hours including nights, weekends, and on-call availability;

Must be available by phone for on-call purposes.
