The Mid-America Regional Council (MARC) is seeking a qualified individual to serve as an Integrated Care Specialist. This person will support multiple community-based services that support health and independence for older adults in the Kansas City region. This position responds to community requests for information and referral, interacts with contract care managers to advance the health goals of clients, serves as a liaison to health provider partners under integrated care initiatives coordinated by MARC, and provides professional assessment of client eligibility and service priority.

About this PositionThe following represents the essential functions of this full-time position.   • Conduct professional assessments of clients, including in-home assessments for Older Americans Act services as well as annual Medicaid Re-Assessments for the State of Missouri.
  • Respond quickly and efficiently to individuals contacting the Department of Aging and Adult Services information and referral line. Individual callers include, but are not exclusive to, the following: older adults, family caregivers, individuals with disabilities, and community professionals.
  • Facilitate the introduction of innovative technology to MARC clients, including everyday technology that relates to various MARC service lines, and medical equipment and technology required by health provider partners.
  • Understanding emerging technology, including developing an aptitude for learning new devices and coordination and management systems is necessary.
  • Identify resources (public, private, governmental and/or for-profit) to accurately refer callers to services, according to individual characteristics and client risk levels.
  • Represent the Department of Aging and Adult services at various community events, such as health fairs, professional networking events, and committees facilitated by MARC or other partners.
  • Arrange for education and outreach opportunities. Education will include external, public education on a variety of topics such as Medicare Basics, long-term care planning, programs supported through the Older American’s Act (OAA), Medicaid, etc. Audiences may include older adults, family caregivers, and/or health professionals.
  • Deliver support, information, advice, feedback or assistance to management, program staff or colleagues to refine work outputs or resolve problems.
  • Make recommendations on meeting emerging needs of older adults in the community and assist in implementing improvements.
  • Serve as an embedded liaison to community services and resources that address social determinants of health among key MARC partners, such as hospitals, skilled nursing/rehabilitation facilities, physicians and other health providers to provide education about the benefits of social health and impact on clinical outcomes.
  • Develops marketing plans, business plans, sales strategies, and action plans for identified targets of opportunity that clearly define objectives, goals, win strategies, schedules, and action assignments.
  • Develops and expands existing client base.
  • Establish and maintains ongoing client relationships with discharge planners, physician staff and physicians, hospitals, nursing homes, residential centers, payers, and other sources as requested by management.
  • Perform other related duties or special projects as requested.

About YouBasic requirements
  • A minimum of a High School Diploma/GED and three years of professional experience in the delivery or coordination of social, protective, medical, mental health, or rehabilitative services for aging adults and/or adults with disabilities in a public or private agency, or closely related, client-centered experience is preferred. Marketing and business experience, especially in the nonprofit sector are advantageous.
  • Once hired, successful completion of the Alliance of Information and Referral Systems (AIRS) Certification Program, within the time frame outlined by AIRS, is mandatory.
  • A Bachelor’s degree in one of the above disciplines can be substituted for experience.
  • The position is required to operate a motor vehicle. Regional travel by automobile in the Kansas City area is expected for this position.
  • Experience with Microsoft Office Suite (Word, Excel, Access and PowerPoint).
  • Must successfully pass background check, including driver’s license check.
Skills you’ll need

The successful individual will possess the following knowledge/skills/abilities:

  • Knowledge of social casework and community resources used in the provision of social services for seniors and the disabled
  • General provisions/objectives/philosophy in senior and disability services programs
  • The ability to conduct assessments and identify risks, assess needs and develop solutions to meet the client’s needs
  • Ability to work effectively with clients, family members, health care providers, community organizations, hospitals and other agencies, MARC employees and the general public
  • Some knowledge of medical terminology related to Medicare benefits
  • Possess strong customer service skills
  • Have a high degree of patience, respect and empathy while serving an aging population
Additional qualifications

  • A Bachelor’s degree in Social Work, Psychology, Sociology, Gerontology, Human Services, Public Administration, Nursing or a closely related field.
  • Current or previous experience in a non-profit setting.
