Intake Coordinator Details

The Intake Coordinator will assist with the coordination of prospective patient's screening, the admission process, and marketing the services of the program to internal and external referral sources.

Monday through Friday, 8:00-5:00

Responsibilities

  • Coordinates patient intake screenings with the clinical staff.
  • Assist with intake screenings as necessary. Discuss result of screenings with Program Director/Coordinator/Nurse Manager, Social Worker and/or RN in the program prior to admission.
  • Follow-up with all referral sources on outcome of intake screenings. If patient deemed inappropriate, referral source will be notified and told reason for not admitting the patient. Medical information will be relayed by Program Director, if necessary.
  • Market and educate program services with both new and existing referral sources on a weekly basis. Provide feedback to Program Director/Coordinator/Nurse Manager on issues of concerns from referral sources. Assure that all appropriate clinical staff address concerns from referral sources in a timely and professional manner.
  • Abide by all hospital, Inpatient and Outpatient Behavioral Health Services policy and procedures to provide quality patient care.
Qualifications

  • Degree in Medical field, such as LPN or higher preferred.
  • Knowledge and experience in Community Relations.
  • One year experience in the mental health field.
  • Excellent Clinical Skills and Judgment
