Job Purpose:

The Bilingual Flex Housing Specialist is responsible for conducting initial telephonic assessments of all new housing referrals that are received from our contracted Accountable Care Organizations (ACOs) for housing support. The Housing Specialist is a member of the Community Supports Department and assists in related activities as appropriate and as time allows. This position will help facilitate housing support services as a recognized social determinant of health to advance the integration of health care and community-based supports.


What you will be doing:

  • Contact referral and schedule an assessment.
  • Conduct a thorough telephone assessment to determine unmet needs, availability of formal and informal supports, program eligibility, barriers to participation, and overall willingness to receive and engage in services.
  • Timely completion of data entry in appropriate databases, which includes demographic data and program/services documentation.
  • Assist member with CORE documents obtainment.
  • Provide member with individual advocacy and support.

What we are looking for:

  • Bilingual in English and Spanish required.
  • Bachelor's Degree in Human Services or related field desired. May be substituted with equivalent experience living with a disability
  • Three to five years of experience in the human service field and experience in coordinating services for one or more of the target populations.
  • Ability to travel independently & reliable transportation required.
