Housing Case Manager


General Description:
The Housing Case Manager provides direct client services to individuals housed with subsidized vouchers, providing case management services and maintaining all documentation necessary to provide conformance with housing regulations. Schedule: Full time. This person reports to the Program Director.


Required Education, Knowledge and Skills Abilities to

Perform Essential Job Functions: A Bachelor's degree is required. Experience in working with special populations is preferred. The ability to work as a part of a team is necessary. A clean driving record with no more than two points is essential. Ability to respect individual choice and commitment to serve homeless and recently housed populations is required. Also, this position requires excellent verbal and written communication skills, the ability to manage people and the understanding to respect the abilities and limitations of individuals.


Summary of Essential Functions, Responsibilities and Duties

  • Support individuals and families in gaining and maintaining housing stability through service plans that incorporate goals for the household.
  • Ensure all documentation needed for Housing applications, placements, and renewals are correct and complete.
  • Provide quality case management services to individuals.
  • Meet face to face with households in their homes to puruse goal related tasks, review progress, and address barriers to accessing services.
  • Network with community resources for housing, employments, behavioral health services, etc.
  • Act as a liaison with landlords and other community supports as needed
  • Coordinate somatic physician, therapy and psychiatrist appointments as needed
  • Responsible for daily, monthly & bi-annual paperwork for individuals
  • Assure accurate and timely reporting of required information and statistics
  • Ensure fiscal compliance with agency equipment, vehicles, and funds
  • Assist with specialized services and needs such as MTA and ID cards
  • Report all incidents to Program Director within 24 hours
  • Ensure individual records are complete and in compliance with all required standards
  • Utilize computer systems for external/internal communication, and Time/Attendance on consistent, weekly basis
  • Provide crisis management as needed.
  • Perform other duties as requested by Program Director
