Position Overview

This position demonstrates competency in delivery of quality patient care to patients ranging in age from infants to geriatrics. The RN Case Manager is responsible for developing and implementing a plan of care in accordance with the patient?s expressed wishes and delegates care and interventions in full compliance with the Nurse Practice Act and organizational policy, procedures and standards of care.

Essential Duties

  • Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients in accordance with physician orders and established policies and procedures
  • Develops and coordinates plan of care in collaboration with patient, significant others, and interdisciplinary care team members to focus on holistic needs of patient in order to assure satisfactory outcomes and achieve patient care goals.
  • Makes nursing visits according to the Plan of Care. Delegates appropriate interventions to LPN and nurse aides. Follows organizational policy at time of death.
  • Provides direct patient care according to the plan of care and in accordance with the Nurse Practice Act and Hospice Standards of Practice.
  • Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
  • Accurately and timely documents nursing assessment, interventions, and updates to Plan of Care according to the nursing standards of this organization.
  • Delegates tasks and supervises the activities of nurse aides and LPNs and documents supervision according to organizational policy.
  • Provides caregiver with appropriate instruction to support safe care of the patient in the home and in accordance with the current plan of care.
  • As a Case Manager, the RN will:
    • Serve as an active member of the interdisciplinary group (IDG) and attend IDG meetings regularly.
    • Complete documentation and re-certification orders accurately and in a timely manner according to standards of the agency.
    • Assist in chart audits to insure compliance with regulations
    • Supervise CNA services provided to assigned patients
  • Maintain confidentiality of patients and their families, co-workers, volunteers, and the organization at all times.
  • Adhere to the organization?s Standards of Conduct and HIPAA regulations.
  • Report any potential misconduct or violation of the organization?s Standards of Conduct, HIPAA regulations, or federal or state laws and regulations.
  • Participates in on-call schedule prn.

Non-essential Duties

  • Other duties as assigned to promote the efficiency and effectiveness of the organization.

Requirements

  • Graduate of accredited school of nursing – Associates Degree, (BSN preferred)
  • One year of clinical nursing experience preferred
  • Certification in Hospice & Palliative Care is desirable
  • Current North Carolina Registered Nurse license
  • Valid NC driver’s license, reliable independent transportation, and current automobile liability insurance at limits required by the agency
