What we’re looking for in you

Required:

• Graduate of accredited physical therapy program
• Continuing education as required by state licensing boards
• Must have MN or WI Physical Therapy (PT) License upon start date
• Must provide proof of MN and WI license within 4 months of start date, if both MN and WI PT licenses are not present on start date
• Ability to read, write, and speak in English language
• Must have a vehicle with auto insurance and a valid driver's license and meet all criteria required under Lakeview's Automobile Liability Guidelines
• Must have current CPR/BLS through American Heart or Red Cross

• Proof of application for alternate state within 4 weeks of start, if both MN and WI PT licenses are not present on start date

The purpose of the Homecare Physical Therapist

As a Homecare Physical Therapist, you will provide physical therapy to clients in their home. You will complete admissions, evaulations, assessments, treatments, and discharges of clients who need physical therapy in their home. This position also supervises home health aides and physical therapy assistants.
