WHAT YOU WILL DO

  • Perform comprehensive speech and language evaluations, treatment and related follow-up services including discharge planning.

  • Document patient assessments and pertinent information in a timely manner.

  • Complete appropriate goal re-assessment and discharge planning.

  • Provide speech therapy, including selection and use of assistive equipment.

  • Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.

  • Provide speech and language therapy according to standards of care defined by the state practice act and other professional and governing agencies.

  • Train and educate patients and family members or others support and document understanding of treatment plan and goals.

WHAT IT TAKES TO SUCCEED

  • Prior Speech Therapy and/or Home Health experience.

  • Customer service skills.

  • Accept personal accountability for deliverables and actions.

  • Effectively communicate verbally and in writing.

  • Critical thinking skills.

  • Deliver patient education under established protocols.

  • Develop effective relationships with patients, hospital departments, and other external parties.

  • Develop own ways of working effectively and efficiently.

  • Handle multiple responsibilities simultaneously.

  • Maintain composure, even in very difficult situations.

  • Show sincere interest in others and their concerns.
