Personal Care Aide

POSITION SUMMARY

A Home Health Aide (HHA) or Personal Care Aide (PCA) is a person who provides nutritional support, assistance with personal hygiene and the environmental maintenance necessary for an individual to remain in his/her own home. The Personal Care Aide is under direct supervision of the licensed nurse. The PCA provides services in accordance with Level I and Level II Personal Care Aide Functions and Tasks Scope of Practice.

Home Health Aide (HHA)/Personal Care Aide (PCA) - QUALIFICATIONS

Successful completion of a New York State Department of Health/Department of Social Services approved Personal Care Aide training program as demonstrated by valid Personal Care Aide Certificate.

  • Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.
  • Ability to speak, read and write in English sufficiently to understand and interpret the HHA Plan of Care, document care provided on the HHA Time and Activity report and able to call agency to report change and /or issues related to the patient and /or 911 in case of emergency.
  • Holds a valid Certificate.
  • Ability to add and subtract two digit numbers and multiply and divide with 10’s and 100’s. Ability to perform these operations using units of American money and weight measurement, volume and distance.

Home Health Aide (HHA)/Personal Care Aide (PCA) - DUTIES & RESPONSIBILITIES

  • Preparing and serving normal diets. Assisting patient with eating monitors intake.
  • Assisting with bathing of patient – in bed, tub and shower.
  • Assisting with grooming, care of hair, including shampoo, shaving with electric razor only, and ordinary care of nails – this means soaking and filing nails.
  • Assisting with care of teeth and mouth.
  • Assisting patient on and off bedpan, commode and toilet.
  • Assisting patient in transferring from bed to chair, wheelchair and in walking with or without devices.
  • Assisting patient with dressing.
  • Assisting patient with self-administered, oral medications that have been ordered by the medical practitioner.
  • Use of special equipment i.e. hoyer lift.
  • Passive range of motion, turning and positioning.

Reporting and change in patient’s mental and physical condition or home situation to the nurse.

  • Making and changing bed/linens.
  • Dusting and vacuuming the rooms the patient uses.
  • Tidying bedroom
  • Tidying bathroom
  • Patient’s personal laundry; this may include necessary ironing and mending
  • Provides a supportive environment and ongoing reality orientation to confused patients using appropriate interpersonal behavioral techniques.
  • Assists with self-administered medications.
  • Administer special skin care as directed.

WORK ENVIRONMENT:Patient’s home, facilities.

EMPLOYMENT REGISTRATION REQUIREMENTS

1. UNEXPIRED - NYS ID, Drivers License, Passport, Permanent Resident Card and/or

Work Authorization Card

4. Two (2) Reference Letters – Professional work letters ONLY

5. Physical - (Not more than 12 months old)

6. Drug Screen – (Not more than 6 months old) (Lab Report with results)

7. Rubella Immunization – must be POSITIVE with Titers on LAB Report. (This Never expires)

8. Rubeola/Measles Immunization - must be POSITIVE with Titers on LAB Report. (This Never expires)

9. Annual PPD results – (Not more than 1 year old)

10. COVID-19 test (either a negative result or positive for antibodies)

** If PPD is NEGATIVE you MUST have result information on Physical form

** If PPD is POSITIVE you MUST have CHEST X-RAY Lab Report

** Chest X-Ray can NOT be Older than 5 years

** If you have a Chest x-ray you MUST have a TB SCREEN QUESTIONAIRE every year.

Example: If your Chest x-ray was taken on 11/21/2009 you MUST have a TB Screen Questionnaire for

11/21/2010, 11/21/2011, 11/21/2012, 11/21/2013 and on 11/21/2014 you will need a New Chest x-ray.
