The LCHSA Home Care Coordinator is responsible for coordinating the care to elders and others who require assistance from a certified Home Health Aide or Personal Care Aide in the activities of daily living. ? Handles multiple and consistent incoming calls, using agency communication template and documenting promptly in HHAX Patient/Caregiver profile.
? Manage client/contract satisfaction with appropriate customer service, communication and follow up.
? Provide day to day coordination and scheduling of assigned patient census.
? Ensures caregivers understand and can fulfill Plan of Care and Attendance Verification requirements.
? Work cooperatively with Management to maintain compliance with State and Federal Regulation standards.
? Responsible for the coordination of daily Caregiver assignments and coverage for sick calls/no call/ no shows.
Fully vaccinated staff, office space that allows for enough distance between desks, office is properly cleaned every day including bathrooms, front desk, and areas of high traffic, walk-ins are required to wear a mask.
