As a Health Link Care Coordinator, you will be assisting clients in the community to find the resources they need. Your work will include encouraging clients to create and track Integrated Care goals. You will work with clients on health promotion and individual needs. You will work with community partners to coordinate and close gaps in care. Am I a good fit? In this position, you may be travelling in the community to meet families where they are. A flexible schedule is important for this role, and allows you to have control over your session planning. Office hours will also be a part of this position. You will have significant documentation, so good time management and organization is a must! Qualifications- Bachelor’s degree is required. 1 year experience with social work or case management desired.
