Position Summary:Works closely with the prenatal health care team to provide care coordination by conducting patient intake, assessments, health education, care planning, interventions, advocacy and follow-up. Provide services with respect for the autonomy, dignity, privacy and rights of the individual. Follows CPSP standards and maintains open communication with the treating clinician regarding patient care.
Essential Functions of the Job:- Conducts initial prenatal intake interviews that include medical history and prenatal orientation. Orients the patient regarding prenatal laboratory tests ordered by the physician and assists the patient in obtaining these tests.
- Conducts psychosocial, nutrition and health education assessments.
- Documents interviews and assessments in the medical record according to CPSP and SYHC guidelines.
- Develops comprehensive service plans for each case based on needs assessments which identifies goals, service needs, time lines, and methods of intervention.
- Conducts health education on a variety of topics related to prenatal care.
- When appropriate and in accordance with Steps to Take guidelines, provides preliminary psychosocial intervention and coordinates access to specialty care.
- Promotes linkages to other health center services. Coordinates access to other clinic support services including WIC, Dental Department, Health Education, Prenatal Classes, Social Worker, Counselor and others as determined necessary. When indicated coordinates referrals to additional health, educational and social services.
- Maintains close contact with the clients throughout their episode of care including pregnancy, postpartum and family planning. Identifies the reason for missed appointments, and assists the client in maintaining continuity of care with appropriate interventions.
- Assists clients with practical arrangements including transportation needs, referrals and specialty appointments.
- Ensures that results of tests and referrals are communicated to the clinician in a timely manner.
- Is available as a contact for client problems, questions and addressing concerns. Receives incoming calls for information.
- Provides copies of client’s prenatal records to delivering hospital prior to delivery, including pregnancy, postpartum and family planning. Identifies the reason for missed appointments, and assists the client in maintaining continuity of care with appropriate interventions.
- Assists clients with practical arrangements including transportation needs, referrals and specialty appointments.
- Ensures that results of tests and referrals are communicated to the clinician in a timely manner.
- Is available as a contact for client problems, questions and addressing concerns. Receives incoming calls for information.
- Provides copies of client’s prenatal records to delivering hospital prior to delivery.
- Maintains appropriate enrollment and delivery records. Assists in maintaining tracking systems and accurate records of patients and outcomes. Assists in collection of statistics by entering information in a tracking database and produces reports.
- Will maintain established productivity of patient visits in accordance with clinic policy and program guidelines.
- May participate in community outreach.
Additional Duties and Responsibilities:- Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
- Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
- Performs other related duties as assigned or requested.
- Maintains program specific records as required; submit reports, including activity, contact and mileage reports in accordance with agency policies and procedures.
- Attends meetings and/or training sessions as required and participates on committees as directed.
- Performs other related duties as assigned or requested.
Job Requirements:Education Required (Minimum level of education):- High School Diploma or GED
- Medical Assistant or Bachelor’s Degree in health/social science
Preferred:RN Associate Degree or LVN.
Certifications/Licenses Required:- Medical Assistant or Bachelor’s Degree in health/social science.
- CPR/First Aid.
Preferred:CPRP Certificate, RN or LVN current license.
Experience Required (Minimum level of experience):2 years in outpatient OB/GYN and/or CPSP setting.
Verbal and Written Skills Required to Perform the Job:Demonstrates excellent oral and written communication skills. Bilingual (Spanish/English).
Physical Activities:Prolonged periods of sitting, and constant walking and standing. Driving and occasional travel required.
Working Traits:Superb organizational ability and exceptionally analytical.
Technical Knowledge and Skills Required Performing the Job: Demonstrated- knowledge of health, social service and programs.
- Demonstrate appropriate communication skills and ability to collaborate with the healthcare team.
- Demonstrate the ability to be sensitive to patient needs and ability to motivate the patient in moving towards self-care.
- Working knowledge of the health care delivery system.
- Working knowledge of computers including Outlook, Excel, Word.
- Ability to work independently and meet deadlines.
Equipment Used:Computer, fax machine, pertinent medical equipment including glucometer, wheelchair, scales, thermometer, blood pressure equipment, etc.
Working Conditions and Physical Requirements:Frequent prolonged standing, walking sitting. Occasionally lifts, positions, pushes and/or transfers patients, and lifts supplies/equipment. Exposed to unpleasant elements (accidents, injuries and illness). Occasionally subject to multiple calls and inquiries. May be required to work evenings and/or weekends.
