MAJOR RESPONSIBILITIES: Facilitates all facets of youth Case Management under the direction of the Family Engagement Specialist. Serve as a member of the Youth Treatment Team (coordinating/facilitating meetings). Maintain Department of Children and Families Group Home requirements; and any other contractual agency requirements. JOB RESPONSIBILITIES/DUTIES INCLUDE BUT ARE NOT LIMITED TO: * All positions are awake, active and alert* Spanish speaking candidates highly preferred. The Case Manager must have a Bachelor’s Degree (social work, psychology, or counseling). Three years’ experience working with youth and families is required along with experience working with the population served (specifically with pregnant and parenting youth and youth with chronic illnesses such as asthma or diabetes). Experience working in a Children’s System of Care residential placement preferred. The Case Manager must have a valid driver’s license, pass a criminal background check, and all other agency requirements.
