To assure client services are delivered and clinical practice is implemented in accordance with established
guidelines and contract requirements and to use the principles of supervision, reflective practice and staff
development to build and model a strength-based, client-centered culture.

RESPONSIBILITIES:

  • Ensure the delivery of clinical services, both core and non-core, are in accordance with DHS and other accepted standards of practice
  • Assume responsibility for staying current with changes to rules, regulations and standards as they relate to clinical practice
  • Lead and collaborate with interdisciplinary teams and others involved in the care of patients to ensure that the physical, psychosocial, environmental, safety and developmental needs are met
  • Maintains a team of qualified Public Health nurse home visitors
  • In collaboration with the Clinical Director, is responsible for the recruitment and hiring of nurse home visitors
  • Plans and implements orientation program for new hires including assuring attendance at all required training
  • Schedules and facilitates monthly team meetings
  • Plans appropriate professional development opportunities to enhance knowledge and skill of team members
  • Provides individual 1:1 nurse supervision and support on a monthly and as needed basis
  • Completes probation and annual performance evaluations. Develops a plan with staff to address performance issues as indicated
  • Makes joint visits to review nursing practice meets MVNA and Nursing quality standards
  • Assigns client caseloads and monitors nurse home visitor’s ability to build and manage caseload
  • Carries out community awareness, support and referrals for family home visiting
  • Is agency representative as requested at community fairs and functions to share information on home visiting programs
  • Maintains internal infrastructure
  • Utilizes existing agency policies and procedures
  • Working within existing budget ensures that teams have infrastructure needs met including space, computers and cell phones
  • Oversees the availability and storage of materials and supplies. Ensures staff are using most current materials
  • Oversees quality improvement and evaluation
  • Oversees and ensures timely and accurate documentation and data entry into PH-Doc
  • In collaboration with Clinical Director reads and interprets quality assurance reports and shares with team
  • In collaboration with Clinical Director implements quality improvement projects based on client and agency outcomes and interventions
  • Assesses staff development needs and uses available data to structure appropriate staff education
  • Create and maintain MVNA’s culture of compassion, ingenuity and optimism
  • Ability to develop and maintain relationship of trust with team members, agency colleagues,supervisors, and community partners
  • Ensure that supervision of home visitors utilizes principles of motivational interviewing, coaching and reflective practice
  • Provide regular case conferences and team meetings
  • Effectively assist home visitors in analyzing complex problems and case situations
  • Develop and implement regular reflective supervision within the program
  • Continuously improves his/her level nursing knowledge
  • Participates in community and professional education

QUALIFICATIONS:

Minimum Qualifications:

  • Bachelor’s Degree in nursing from a university or college program accredited by the National League for Nursing or American Public Health Association; at least three years of staff and/or supervisory experience in Public Health Nursing

  • Master’s Degree in nursing from a program accredited by the National League for Nursing or American Public Health Association with includes preparation in supervision preferred; at least three years of experience in Public Health Nursing under a supervisor

  • An approved equivalent combination of education and experience

  • Must be able to work well as a team leader/member. Must be well grounded in professional ethics. Must be able to speak and write the English language in an understandable manner. Must have computer skills necessary to accomplish requirements of position, including email, word processing, related facility software and internet research tools
  • Must possess speech/sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must be able to sit, stand, use hand to finger, bend, lift, push, pull, carry, reach with hands and arms, move intermittently, walk up, and down stairs or use ladder occasionally

  • Skilled in overseeing the efforts of individuals within the department

License/Certifications:

  • Licensed as a registered professional nurse in Minnesota and eligible for certification as a Public Health Nurse in Minnesota
  • Current CPR certification
  • Valid MN Driver’s License and daily access to a reliable and insured vehicle.
